4 months I have been dealing with a bill that should've been paid by the insurance company but instead they need documentation from the OB/GYN Drs office because they didn't like the diagnosis? The documentation was sent over May 26th while I was on a 3-way call with Dr office and Empire. Now to be told today after 5 months and now the bill is in collections, that they sent office notes and Empire does not want that! Why do these places then not contact the providers office back to tell them that's not what we need and what they are looking for? I've never in 27 years of having this Insurance have had any issues until this new year 2022. Why is my claim needing further documentation when I was at an OB/GYN office getting a private personal exam completed? I am a woman, and I don't get up on an exam table for the heck of it for the Dr. to take a sample and send to the lab to see what is going on for the heck of it. I feel very violated that they would need to question why a Dr. took a sample to send to the lab.

Desired outcome: I would like the claim paid and not have any collections in my name. I also would like to have an apology from them for questioning such a claim.
