I was given this combination of pills after being ripped off my psych mess for ocd that had me stable for the prior 10 years. I was complaining constantly about my symptoms and that I didn't feel well and the new doctor doubled my dose of the meds sending me into a syndrome that is not only life threatening, but detrimental for a person like me. While in the throws of the medication induced toxicity the doctor called cps and the police. I didn't have my child, and after all the damage, he dismissed me from his office and sent me to another provider. But before I was told to go he sent me packing with a three month supply of the deadly mess. I told the advocates and they never got back to me. It's been months and I can't get a treatment plan. Because of this, I lost my kid. Shameful.

I accompanied my husband to his doctor’s appointment and the doctor was not his primary one due to his primary not being available. He came in for complaints of a headache that’s been radiating to his ears and eyes. He did not get ANY help from this PA. She came into the room MAD, did NOT greet him or myself and went off assuming things about him Saying he needs to see his primary due to everything he has going on. This office has been treating my husband like sh**. Every provider he’s seen has brushed him off and not take his health seriously. He’s constantly having to fight for himself all the time. We try really hard to be patient, understanding and kind to these people but they lack anything relating to patient care. Absolutely disappointed in the care he’s been receiving and horrified that these “doctors” are allowed to see and treat patients with this type of “caring” attitude. They don’t know what’s going on and for them to assume things is absolutely disgusting. Be respectful of patient’s choices and treat them with care and compassion like you vowed to do!

Halyard location for dental does not answer the phones nor returns voicemails. They are severely backed up due to poor choice on scheduling and time management. A “checkup” exam is 2-3 months out, and from there they schedule your routine schedule the same amount of time out, regardless if you’re a new or existing patient. The staff is never pleasant, helpful or thorough, and from a patient standpoint, this makes for bad practice and standard of care.

i was seen by feroze assadi at the franklin office on january 11, 2022 he gave me medication and i was concerned about how they were working and what was still going on and this man refused to respond to any requests that were put in for a call back or respond to messages sent via mychart as they were read and never responded to after several urgent messages were put in i will not be seeing this provider anymore and don't think anyone should he is very negligent and unprofessional

This incompetent NURSE PRACTITIONER has absolutely 0 empathy for people with pain. He is a scared little man when it comes to prescribing anything for pain as he says it can become addicting. I suffer from CHRONIC pancreatitis. He didn't care. All he could see was my PAST. it didn't matter to him that the medication would be given to me. It didn't matter that I was constantly in pain. No, he's more worried about his clean deathless record. He's rather me go to the ER EVERY TIME I have a flare up. Incompetency. Get rid of this guy. Seriously. If anyone is trying to go to him for pain, GO SOMEWHERE ELSE.
