I humbly refer to the above matter.

I am the husband of the above named deceased, who has passed away soon after delivering a baby boy on 6.5.2012 at Pusat Perubatan University Kebangsaan Malaysia (PPUKM), Bandar Tun Razak Cheras.

My wife was a very healthy person prior and during pregnancy, further the doctors and nurses who attended her during monthly pregnancy check-ups confirm her health condition and she is fit for a normal delivery and as scheduled she was admitted on 5.5.2012 at about 11.30 pm.

Sadly on 6.5.2012, I have lost my loving wife soon after delivery and my son lost the mother without feeling his mother’s hug and love.

PPUKM after the death of my wife has furnished a Summary of Case which created an unsatisfactory curiosity and suspicious.

Subsequently, I have written an email to the Prime Minister Office on 18.5.2012 drawing their attention to the chronology of events that took place during and after delivery until I lost my wife to seek fair and natural justice.

Sometime on 12.6.2012, PPUKM called for a meeting and a Specialist Doctor (whom I am uncertain whether she has attended my wife prior, during and after delivery as well in the Operation Theatre) informed me that an anesthetic was given to my wife in the Operation Theatre room and from there onwards all her pulse, heartbeat, blood pressure dropped and the Learned Doctor had also informed me that the uterus was removed to safe my wife’s life unfortunately she was pronounced death. I had requested for clarification from the said Learned Doctor for reason on the fact of the uterus removal was not made known to me during the death. The Learned Doctor just replied that the Doctors who attended my wife in the Operation Theatre had forgotten to inform me of the same during the time of her death.

On 15.6.2012 PPUKM had furnished a report which is totally very simple with no details clearly stated therein and extremely lay where any layman can produce such a report. In the report it is concluded that the cause of my wife’s death is due to a suspected amniotic fluid embolism. It is also alleged that myself and my family “refused for a post mortem”.

I strongly denied the above statement in my email dated 28.6.2012 that I and my family never at any material time refused for a post mortem. As any lay person we just proposed not to have a mortem but the learned hospital should be aware of the necessity and should have advised the concerning parties of the importance, which they didn’t.

PPUKM refused and/or neglected to produce the actual satisfactory reason for my wife’s death. I am seriously depressed of losing my loving wife and encountering difficulties is raising up my son as a single father. The whole family tremendously lost her.

I have written many emails to PPUKM to give me a fair explanation for my wife’s cause of death and have also written official letters to PPUKM to furnish me timing for all the chronology of events that took place after the delivery of a baby and in the Operation Theatre and also for a pathology/lab report in particularly uterus examination report. However until to-date PPUKM failed, neglected and refused to reply to all my emails and letters and/or furnish me with the relevant requisite particulars.

I felt doubt and suspicious on my wife’s death based on her perfect health condition prior and during pregnancy and this is totally contradicting with the Summary of Case and Medical Report that were furnished by PPUKM and also to the statement given by the Learned Doctor during the meeting and therefore I have sent few emails expressing the doubtfulness and un-satisfaction to seek a fair and natural justice. However until to-date all my emails went unanswered.

On 29.8.2012 and 18.9.2012, I have written official letters to PPUKM to furnish me with the following:-

1. timing for all the chronology of events that took place from the delivery and in the Operation Theatre until I lost my wife; and
2. pathology/lab report in particular on uterus examination report and/or uterus removal report.

PPUKM has replied my aforesaid letters on 25.9.2012 without furnishing the requested chronology of events and reports but merely making reference to the Medical Report of which is very basic and lay.

Accordingly, I have on 22.10.2012 replied to PPUKM expressing my un-satisfaction.

PPUKM have used a force delivery on my wife by using Kiwi Vacuum on few attempts and following with Forceps one after another which caused non stop bleeding rather than going for a caesarean which could have safe my wife’s life.

Enclosed herein some relevant findings and articles on Amniotic Fluid Embolism and collectively marked as “Annexure 10” and the possible cause to patients on such treatment using Kiwi Vacuum and Forceps one after another, which clearly stated that any learned doctor cannot and should not use all these methods of treatment one after another during delivery which may result in death due to a pressure gradient from uterus to vein.

The learned doctors should be well aware of the reasonable cause of action to be taken on delivery and not by using a wrong treatment which will definitely cause death. The doctors should get well prepared to face any kind of complicated cases and not by proposing any possible treatment on patients fighting for life.

All these articles clearly indicate that the methods of treatment adopted by the learned doctors caused my wife’s death due to non stop bleeding.

Soon after delivering a baby boy, my wife stood up from the bed though she was very weak, hugged me and kissed the baby and the doctors who attended her informed me that my wife is encountering non stop bleeding and need to be taken to the Operation Theatre.

The PPUKM attendant facilitate my wife with the Operation Theatre bed, she got down from the labor bed and walked onto the Operation Theatre bed. Thereafter they brought her into the Operation Theatre.

After half an hour later in the Operation Theatre, one of the Doctors came out and informed me that my wife’s condition is very weak whereby her blood pressure, heartbeat and pulse dropped very low and informed me that they need to remove her uterus due to a non stop bleeding.

Subsequently, another half an hour later, the Doctors reluctantly informed me that they could not safe her and I have lost my wife.

The doctors failed to give me a proper advice on the need for a post mortem and the requirements of same and just asked whether I want to take my death wife home.

Within few minutes my wife’s death body was wrapped and sent down to the mortuary to be taken home.

I badly missed my wife and would like to seek a fair and natural justice.

In the above given grounds, I pray for the following:-

1. special and general damages for all the losses suffered;
2. cost; and
3. such other order and/or relief that the Honorary deeds fit

Untuk nurse di WAD5D. Tolong lah layan patient elok elok. Kita ni bukan sengaja untuk masuk atau datang ke hospital. Jadi sebagai nurse dan itu tanggugjawab awak semua untuk buat scop kerja awak tu dengan bagus. Bukan tukar cadar tukar itu datang hantar dan kita yang buatkan sendiri. Kita bayar untuk awak jaga buat skop kerja awak. Bukan datang secara percuma. Muka tu tolong senyum sikit. Kita senyum balas pun tak. Tahu penat sbb kita pun bekerja juga. Duit makan minum nurse tu kita warga patien ni yang bayar. Jadi tolong lah bersikap tanggugjawab sedikit ye. Cakap tu tolong sopan sedikit. Sebab kita pon bercakap dengan sopan juga. Terima kasih!

We hope we are not disturbing your peace since your complaint is more than 2 years old. But we are having to have our negative experiences with Hospital Universiti Kebangsaan Malaysia (HUKM/PPUKM) looked into by The Ministry of Health (MOH) of Malaysia, The Ministry of Education (MOE) of Malaysia, and The U.S. Embassy in Malaysia, and it's quite an annoying situation. Not to mention ridiculous and surreal. The incompetent practices and unprofessional attitudes of most of the doctors and nurses we encountered at HUKM/PPUKM was bad enough from the very beginning of my wife's chemotherapy treatment (for Stage 4 Diffused CD20+ Non-Hodgkin's Lymphoma) there that, I had to have a total of five/six meetings with HUKM/PPUKM's hospital administration/customer service rep - meetings lasting anywhere from 30 minutes to 2 hours. After more than 2 months of waiting for some form of resolution to poorly-trained nurses, doctors, and other departments of their staff, I finally wrote to the MOH and The U.S. Embassy in Malaysia to get some satisfactory response to many very troublesome practices and conditions at HUKM/PPUKM. The day before my wife was supposed to continue her 2nd cycle of chemotherapy treatment, one of the doctors told her they did not have the Rituximab we were told was going to be donated for her treatment. Within 24 hours of the scheduled chemo treatment, it was nowhere to be found, and the docor - nor anyone else at HUKM/PPUKM - could explain to us what had happened to it. The very next day, my wife was kicked out of the hospital. To make a short story shorter, the management at HUKM/PPUKM just did not want to deal with complaints that could very easily have bene remedied, but they were too lazy and ignorant to follow through. One of their excuses was that my standards are different than theirs. Well, for me to have to request for HUKM/PPUKM to have an exterminator to spray for ants and mosquitoes in the bathroom of the isolation room my wife and I were staying in at HUKM/PPUKM - the fourth location of the cancer ward we were switched to - was a disappointment. So was requesting that someone come up and get rid of a (live) mouse that I caught in the same isolation room. Then having to go make an additional complaint to HUKM/PPUKM's customer service department, because the mouse feces (aka "poop") that was supposed to be cleaned up the day before - as well as the entire room - was still in the very same spot on the floor. How do two adult males with brooms and mops spend 30 minutes to an hour in a room to clean it, and miss mouse excrement? I thought it had to be a scrape on the tile floor. I could go on and on, but I won't, because the list of discrepancies is way too long to go into without boring everyone to tears. The fact that I was threatened with a law suit by one of the doctors, and we suspect HUKM/PPUKM is trying to sweep these matters under the rug to avoid undesired exposure by attempting to sue me as well. Fact is, if the management of HUKM/PPUKM had done their job to begin with, they'd be spending much less time, money, and resources defending the non-defendable. I tried to help them see issues they would otherwise not be able to observe, and things started out quite friendly (on our parts), but they allowed their cloudy judgment to turn things into something harmful to many people's careers. Maybe this is God's way of cleaning house. Too much bad karma has been allowed to congest the arteries of productivity and prosperity at HUKM/PPUKM. And I guess it's time to get rid of those individuals who have been too lazy for too long and allowed too much damage to be caused to the facility's policies and procedures, and the regulations set forth by the Malaysian government to keep a "tidy house". Anyway, we are having to find another hospital to get my wife treated properly. Since it has been almost 2 months since her 2nd chemo cycle was discontinued due to her catching a bacterial infection - not to mention an E. Coli infection - at HUKM/PPUKM. We recently found out that a week after my wife was admitted to HUKM/PPUKM's cancer ward that, she had worms in her blood, but the doctors did not mention this to us at all, and my wife was never given an anti-worming medication - which a hematology (blood) specialist prescribed directly after the consultation, which she noticed the worms reference within seconds of reading HUKM/PPUKM's report where their lab had indicated that my wife's blood was positive for contaning evidence of worms. My wife's remission won't last forever, so we've got a PET Scan appointment scheduled for this coming Thursday. Anyway, God bless you and your family. And if anyone else has had negative experiences with HUKM/PPUKM, please feel free to contact us, so we can bring the parties responsible to justice, before anyone else is subjected to the poor politics of HUKM/PPUKM's management staff, and all other parties who are suspect.
