non payment of my e-dividend

I got the E-dividend registration form, completed duelly and forwarded it to your Lagos office since 2017 but till date I have not received any correspondence from your organization neither have I received any E-dividend from your organization. I would want my E-dividend yo be paid as soon as possible. A copy of my form is hereby attached for your perusal and immediate action
