This belt doesn't sag or twist even with my Taurus g3c, which is my main carry weapon, and a heavy offset holster. My CZ 75b, which is even heavier, didn't sag it either. I tried every carry position including cross draw, appendix, and small of back, and it handled them all with ease. Whether I am wearing IWB or OWB, this belt is perfect for all gun styles.
