I am on disability, and the Waiver of Premium on my employee life insurance policy was denied.
If approved, this would have continued my coverage that both myself and Humana had paid on for many years - both group and Individual policies.
Denial was based on one phrase and it was completely taken out of context. All other information was disregarded. I requested all forms and documents l, and I have good reason to believe they were dishonest about what they did/did not receive, or perhaps just didn’t review all pertinent information.
Appeals process was not helpful and ignored the concerns in my appeal.
I see a lot of providers, and I understand it complicated the review, but this should not have been a problem for a large health insurance company. I was misled when I was told to take forms to all my providers, when they already had the information from my pcp that had taken me off work. The denial was based on one of those forms, from a doctor I was no longer seeing.
I would also like to point out that the initial letter and forms received to request this waiver all had incorrect contact information on them - addresses, email address, phone and fax numbers. It took several months and many phone calls and e-mails to get the information to the right place (as well as my provider’s).
I had to deal with this, as well as numerous requests for information while I was going through and recovering from major surgery, multiple medical appointments and procedures, and the acute and devastating illness and death of my brother.
I have many serious, ongoing health issues, and dealing with this whole issue has been detrimental to my physical and mental health.

Desired outcome: My only option left is to seek legal advice, which I would prefer not to do. I would like this decision to be carefully reviewed and reversed.I would appreciate a response as soon as possible.
As of today, I have not received any response from Humana. Since I have proof they did indeed receive the proper requested information, I will be considering legal action.
