Physician FML form denied.
HR violated the FMLA by refusing to accept the FML form provided by the Physician.
Employee received mark ups for absence during a FML emergency.
Caroline in HR refused it. She stated that every day absent on FML requires a doctor excuse. She stated there should be an excuse provided for each day on FML.
The physician provided the FML form with the dates, employee received mark ups on those dates for being on FML.
The FML form provided by the physician was disregard and denied.
This is a HUGE violation of a Federal Law.
Desired outcome: Train HR department to adhere to Federal and state laws.
