Sleep Country Canada - gift card 15%

I was informed that a15% gift card would be forwarded to me when I purchased two twin beds on the 12th of Feb. and came to this sight to find the form to apply. Where are the forms. I would very much like to file as this is quit a lot of money that I could use towards further purchases in the store.Thank you in advance for your earliest help in this matter.
