Hello, I just purchased a mattress and a Lifestyle Adjustable bed frame from the Abbotsford BC Sleep Country store and was told I would receive 15% back in the form of a gift card. I need to fill out the application on line. However I cannot find the application anywhere. Can you help me with this?

I will be very disappointed if this does not come through. My experience in the store and with the salesman were awesome. Please don't let this issue make a great experience become a disappointing one.
