I am writing regarding multiple dates of service which I would be happy to send you on 2 patients that are not being paid correctly or at all...
the patients are married and they are covering each patient differently even though they have the same insurance...
EX: both patients had a physical and they denied the office visits on both patients but covered an x-ray on 1 but not the other, both had a colonoscopy and they covered the 1 patient but not the one w/ history of polyps and family history.

We have appealed this multiple times and they say it's not UHC fault it's optum fault, but they say it UHC denying... now they say we are at the final appeal and will not pay...
I will be getting in touch with the department of insurance regarding the issues with these claims...
