Went in for dental cleaning and overall checkup. Was told my insurance did not cover anything. My insurance only provided a discount. Not sure what my insurance was billed for.

Financial adviser asked what I wanted done that day and I answered only dental cleaning and the two issues with 2 teeth addressed. Adviser made me sign without my glasses and without explaining anything.

Dr. started treatment on my teeth without explanation. Next thing I knew the Dr. was putting injections into my mouth. Then the assistant was putting purple stuff in my mouth. Bottom line they had made me sign for a treatment I had not even asked for. Ended up with an invoice for $1700.00, 150 out of my atm debit. A personal loan for $1500.00 which I did not apply for.

Bottom line the financial adviser knew what I had asked to have done and she still committed fraud by not explaining and allowing me to sign without my glassed. The paperwork she had me sign was prepared in advance and both her and the Dr. knew what they were doing.

Office is committing fraud by providing services not asked for and obtaining personal information on the forms like SSI to apply for loan without consent.
