To whom it my concern,

I would like to request for cancellation of my Family Protect insurance under my name. Due to financial Problem i can’t pay any more the monthly charge.
Actually i called hot line of Bankard to cancel my credit card but they told me to inform Family protect to cancel my family protect insurance first.
Account name Noemi Mabita RCBC Bankard ending number 9019.
Pls. Inform bankard for cancelalation of my family Protect insurance.
