I paid Northlake Gynecology at 7777 Forest Lane Suite 324C, Dallas, TX 75230.
When I reced smart Summary from Humana I found that Humana had paid the entire bill.
-I have contacted this office several times and the do not respond. I even talked to a lady by the name of Mercedes. I talked to her twice and she promised to get back with me.
-I am owed $156.14.
-Please tell me what else I am to do to collect this overpayment.
-I can not get my documents to copy, but I can mail you copies.
