Fifth Third Corporate Office review: Request a 1099 tax form

My name is Sherry Craft. Last year the Children's Home Society of Florida forced all members of their retirement fund to take a forced payout. I still have not received a 1099 tax form. To make matters worse, I have been calling Fifth Third Bank's corporate office in Cincinnati Ohio since February 8th. I either get passed around to the next associate that has no clue how to access the information, put on hold, or after waiting for an hour, hung up on, or promised a call back that never happens. This is simple.
