Arizona Diagnostic Radiology review: MRI exam lost

I've had 3 MRI's done with this company all within 2 months. The second time I had the MRI taken it was done incorrectly due to not recording the brain/spinal fluid which was requested by my neurologist. The third time was an attempt to fix this MRI and record the brain/spinal fluid, however the exam was lost and they are now having me go back a fourth time to redo it. They could not exactly tell me what happened with my examination.
