Gold's Gym - harassment, hostile work environment and then terminated

Several Instances have been reported about the Manager Brian harrassing me at work, calling me names, making me cry, and writing me up for everything. Schedules me intentionally on my unavailability times and I was told that if I didn't work I would be fired.
I was told by several other employees that he targeted me because I am a girl and he did not like my boyfriend. Who was also harassed at work until he quit.
I was told that we both are being black listed from the gym so that we cannot work out there even though we hold a membership. He does this to every employee that leaves. Several over the past few months have left due to his behavior and name calling. We are called idiots, he told me he could not stand me being there, and several other issues.
We have contacted the DM but nothing was done. Yesterday he fired me for being 5 min late because my car broke down and I had to get a ride. He wrote me up for that also.
