" I want to file a FORMAL COMPLAINT " with regards to Lincoln trails in Radcliff ky

I was contacted to pick up my child immediately with living 2 hrs away after she was violent towards a peer for calling her a [censored] and after attacking a nurse.

Hence, They discharged a child who was dangerous to others back into home and into community with me having to be in a car with her for 2 hours. She could have been violent towards me while driving. However come to find out she had received some form of chemical restraint to where she was too sedated to do so.

This is after med changes have been made. I am aware that psychotropics in teens can have opposite effects. So they've released a more aggressive child after med changes that could have made her more irritable and aggressive. Because dekayla has never had aggression nor attacked anyone prior to being admitted. She was recently raped and attempted suicide. So being restrained could have been a major trigger or exacerbation of aggression and why she couldn't calm down due to the restraint because Aggressive behaviors was not a behavioral issue upon admission.

I was informed she got a shot after being restrained. She was released to me with no knowledge of what was in the shot nor was I educated on what side effects to look for from the shot. She was heavily sedated when released to my care and seemed not properly observed to ensure no issues from injections would occur.

This is concerning because while she was there she has convulsions from medications which sounds like she actually had Extrapyramidal Symptoms (EPS). This is a reaction one can have from antipsychotics. If this was possibly part of her injection prior to abrupt discharge, I was not informed. With her difficulty from meds, I should have been informed and educated on it from nurse or doctor and I wasn't and she was to medicated to do so.

I took my child there for help and they discharged her with verbally knowing she had increased aggression for which they physically and chemically restrained her and discharged her home less then 6 hours later.

This is knowing that I was fearful of being unable to keep her safe. If they couldn't keep her safe how am I going to do so.

This is after referring her to a higher level of care ( RTC ) that they are telling me is a 3 month wait. They know she needed more care but they sent her home overly medicated with less care and more trauma at the hands of the facility.
