SUBMIT A COMPLAINT / Poor service and poor customer service

I purchased unlimited minutes for 99. 99 i may 2014, found out later that i can only use one phone. So i called back to cancell my order and get refund back. I was told that i will get refund back within 48hrs on credit card on file. But i never did get refund back. Since last 3 mths i call them back every 48 hrs. Everytime i call, same words and nothing happens. I ask for complaint # and confirmation for can ellatio. ,. Ii never get that. Also i asked everytime for superviser, i get she is on the other line. When i say i will wait, the release me from line
Overcharge billing i complain by e mail and phones following persons give me promise to solve my problem with in 48 hrs but no one does look into it.
