They refuse virtually every claim submitted for any number of reasons. No matter how many times you submit the documentation they ask for they refuse it for another reason.

They intentionally "run out the clock," meaning they keep denying claims until they get to keep all of the money myself and my employer paid into it, and we get nothing.
I submitted my receipt for my partial payment of a hospital stay that cost over $45,000 and they denied it. This claim is also going to the BBB and my state board of health and Ohio State Attorney General.
