I should have done my due diligence online before ever accepting a project from this retail maintenance company working on Gymboree in Osage Beach MO . A quick Google search shows MULTIPLE websites with the same complaints about this company. We will be contacting the retail store directly to find out if they have a phone number that actually works for Advanced Facilities Maintenance Corporation 6171 Huntley Road, Suite G, Columbus, OH 43229.

We hated to do it but finally went to the Retail Client and told them of our situation. They did some digging and told us that AFMC had already been paid for that work order. Later we were sent information that Jack Harris from Intercept Management is in charge of the receivership handling Advanced Facilities Maintenance’s debt. From now on we will not hesitate to go straight to the retail Client! We did not receive this notice as a creditor and would have been out of luck and stuck with unpaid invoices!

Here is the info we were given, I hope that it helps: .

IN THE COURT OF COMMON PLEAS
FRANKLIN COUNTY, OHIO
In re Dissolution of :
: Case No. 15 CV 10010
Advanced Facilities Maintenance Corp :
: Judge Reece
NOTICE OF APPOINTMENT OF RECEIVER AND
DEADLINE FOR FILING A PROOF OF CLAIM
December 1, 2015

To: All Creditors of Advanced Facilities Maintenance Corp.
On November 25, 2015, Advanced Facilities Management Corp (the “Company”), was placed in
a receivership for the benefit of creditors by the Franklin County, Ohio, Court of Common Pleas in Case
No. 15 CV 10010, the Honorable Judge Reece presiding. Jack Harris of Intercept Management Corp.,
has been named by the Court as Receiver. The undersigned is the court-appointed attorney for the
Receiver. The purpose of the receivership is to liquidate the assets of the Company in an attempt to
maximize a return to creditors. At this time, it is premature to determine the course that this matter will
take and the final results that will be achieved.

Court Documents:
The order appointing the Receiver, and all other documents for case are available from the Clerk
of Courts, Franklin County Court of Common Pleas; 345 South High Street; Columbus, OH 43215.
Certain documents may also be available online at the Clerk’s website: http://fcdcfcjs.co.franklin.oh.us/CaseInformationOnline/. Please do not contact the Receiver or the
undersigned for copies of court documents. You will be directed to the Clerk of Courts for copies.

Company Information:
The Company is located at 6171 Huntley Rd. Ste. G Columbus, Ohio 43229. The business has
ceased operating and the Receiver is liquidating the assets.

Proofs of Claim:
Creditors who wish to share in any distribution of funds at the conclusion of this case must
submit proof of claim within forty-five (45) days of the mailing of this notice. The order appointing the
Receiver permits the Receiver to set a deadline for proofs of claim. In order for your claim to be
considered, it must be received by: January 22, 2016

Creditors who do not submit a proof of claim on or before this date might not share in any
distribution at the end of the receivership case, if there is a distribution.

Please note the following in relation to submitted proofs of claim:
1. Please use the attached proof of claim form. Print or type responses.
2. Do not submit proofs of claim to the Court. Submit the proof of claim to:
Intercept Management Corp
c/o Jack Harris, Receiver
85 E. Gay St. Suite 1000
Columbus, OH 43215
3. Proofs of claim should be notarized.
4. Enclose supporting documentation for the claim, such as the last invoice/statement for
services sent to the Company, or copies of leases, etc.
5. The Receiver will not acknowledge receipt of proofs of claim. It is strongly suggested
that you return your proof of claim form by certified mail.
Case Developments and Further Inquiry:
The Receiver is required to report to Court from time to time and obtain approval for taking
certain actions. Typically, only parties that have specifically requested notice in writing or that are
specifically impacted by certain proceedings will receive copies of ongoing court documents. If you do
not request notice, you will be informed only of major developments in the case, such as a proposed
distribution.

If you have any questions, please send them in writing or by e-mail to the address above for a
quick response. Due to the large number of creditors in this case, it is clearly burdensome to attempt to
handle inquiries by telephone. We appreciate your cooperation in this regard.

Sincerely,
s/ Myron N. Terlecky

I agree with the others! It's been 10 months of run-around and we still haven't been paid for the electrical service work we did at Gymboree Kitchener (Ontario Canada). I just sent the invoice to our collection agency this morning and have been calling Gymboree directly, and then found this information. Thank you Reliable Contractor! This helps a lot!
