Greetings to you sir/madam, I am the wife of deceased Charles E. Brantley, my name is Janet M. Brantley Plaintiff. I petition your service that was given to my husband {Charles E. Brantley} on or about 12-11-2019 at 15100 Crosswinds apt. 701. Which led to his death quickly within hours upon your arrival after leaving Houston Northwest Hospital with doctors' orders in turn you as Defendant A Hug away healthcare, in turn took upon yourself to administer your own medicine a total of 10 which I never recall deceased Charles E. Brantley taking while hospitalized. And led to cause him to begin to hemorrhaged uncontrollably from his genial {his penis and rectom} which Charles E. Brantley had to be rushed back to hospital, within a few hours upon defendant's arrival.

As plaintiffs husband Charles E. Brantley laid there helpless to no avail, ER doctors had to say he had less than 12 hours to live again due to defendant's negligence as Plaintiff Janet Brantley watched scarcely. For these reasons I the wife Plaintiff Janet M. Brantley is asking $4,000.000.00 {4 million} in damages. Defendant sent employees name Anthony which was very out of control and immediately I knew something was wrong. At the same time Linda arrived as Anthony kept giving her all these different medicines to administer to my husband Charles E. Brantley who's deceased due to this. I petitions defendants who states she wasn't told what to do for these actions plaintiff Janet petitions defendant $3,000.000.00 (3 million}, in damages that led to a early death within hours in the home.

The medicines administered by defendants employees Linda, and Anthony caused Charles E. Rantley to do unusual things he had not done doing his hospital stay. I again questioned Linda what are you doing and why, but to no avail. As I questioned Linda, Anthony would circle apartment with more medicine. Defendant A Hug away {owner and some staff members} arrived at hospital December 12, 2019 asking please tell us what happened. Plaintiff ask you all questions, please don't come in and why are you here. Plaintiff informed defendants I will tell you soon as Charles E. Brantley laid to take his final rest with family by his sides, gone too soon due to defendant's neglect. Plaintiff petitioners are asking $3 million for these actions.

As a result my husband is gone due to your negligence too soon, and for these reasons I plaintiffs, petitions defendants A Hug away and staff for the amount of $5 million and all monies in damages, done to Charles E. Brantley also plaintiffs pain and suffering, plaintiffs asking $4 million and all monies..

Please do whatever is necessary to address this problem as soon as possible. If the problem is not corrected within 5 days days, I may pursue legal action to protect my rights under the terms of applicable law.
