In December of 2021 I started seeing Dr. Ali Raja at Neurosurgery Specialist in Little Rock , AR 72205. They scheduled me for several scans & MRI. On my doctors visit on 3/4/2022 I was given my results at the time of my office visit. Dr. Raja told me that I had a condition that he was surprised that I wasn't already paralyzed & that I really needed surgery to try to have some type of mobility. My condition was severe & that even with surgery I still Have a 50/50 chance of becoming paralyzed if I lift more than 5lbs, no pulling pushing, moving things, riding on any equipment for any length of time is really out the question. On that day Dr. Raja took me off work & stated that I couldn't return to work & he did supply me with the documents needed for my employer. A few weeks later I had surgery on my neck & had over 44 staples. They kept me over night & then I was released home. Since my surgery I have kept ever appointment, has every test they tell me & I have gone thru therapy & I am currently going to pain management. Now at this moment I have been off work over 8 months with no income. I did pay in for long term disability at my job when I was working but when I sent the documents to the Dr. Office I was told that they do not fill out the long-term paperwork & I needed to see my primary care physician. Upon taking documents to my Primary care physician, I was told that they do not fill out the paperwork either & that it was not their responsibility. They were not the one to so the surgery. So Then I contacted Dr. Raja office to inform them of what I was told & they still tell me it's not in their policy to do that paperwork. I am still a patient at Dr, Raja office & I was recently seen at his office. I was also told that he would see me back in 2 or 3 months. I was never released to return to work or even told that I could go back. Now I'm being told that the decision will be up to my primary care physician, but they do not treat me for that condition. I started seeking help for my inability to hold things in my hands, severe pain in my neck, back, hip, & legs. I also had been falling, unable to stand for periods of time, Losing my balance. Since the surgery, I'm still falling, having trouble getting around, & trouble functioning doing normal daily activities. My complaint is that I followed all of the instructions given to me and I am now going back and forth trying to get help with paperwork needed to be able to take care of my home. Can someone tell me what I need to do in order to get help. What is the procedure & policy for caring for a patient that has Medicaid insurance as far as seeing them thru their care. For myself I don't want to fill like now, I am falling thru the cracks, my needs are not important, My overall care. Someone has to be held responsible & not just keep passing me along or pushing me to this person & that person without a Resolve!

Desired outcome: To get the needed documents to file for help and to continue receiving the best possible care without be pawned off on someone else that tells me it's not there responsibility.
