Medical Office Billing review: I would like these charges dropped and an apology.

On 09/02/2022 I was seen by Dr. Thomas Schwartz of Eye Care Associates of Sarasota to have cataract removal from my left eye. On this date I was seen by Dr Schwartz in his office. At that time, I signed a form stating that I would be responsible for cost of services obtained through co-pay. The surgery was performed and was successful. On 08/02/2022 I had the cataract removed from my right eye (also successful). On 11/15/2022 I received a bill from Dr Schwartz's office along with my EOB from Medicare and Tricare. On the Medicare EOB for 09/02/2022 the procedure 92025 (Corneal Topography) was identified and denied claim as not deemed "medically necessity". This same statement was identified and claim denial for the same reason on 08/02/2022. On Eye Care Associates of Sarasota statement, the cost of this procedure was placed under Co-payment for the amount of $50.00 for each date. I went to the office and attempted to speak to the billing person, who came out of her office long enough to tell me that the form I signed gave them authority to bill me. I understand that if a charge is changed, or denied by Medicare, that the office cannot just add it back to my bill under Copayment. Therefore, I would like to file a formal complaint against this provider/office for these charges
