Talked to Jennifer on phone after being referred for dental insurance by someone we knew. My husband asked repeatly if they would paper bill us after initial payment for insurance. He was told they would and he told them to call tomorrow as he would talk to me about it. Jennifer called the next day and I once again stated that we did not do monthly electronic payments except through our Bill Pay account at the bank and even then it was only when I went into Bill pay and set the payments up for a one time payment at the 1st of each month. She said that wasn't a problem after the 1st initial payment so after asking my husband, while Jennifer waited on phone) we agreed to a one time withdrawal with the promise that from now on they would send us a bill each month. It was another month before we received their member book stating they did not do paper billing. In the mean time Jennifer gave me a number for membership to use until we received our cards through the mail which are attached by push out in the membership book.
We would have never agreed to a monthly payment drawal through our bank as we are 61 and 64 years of age and very careful about fraud. The only way we ever pay a bill is by mail, Paypal, credit card or through our Bill Pay which are all initiated by us.
Today I noticed while online with our bank that there had been another payment made to Formulated Dental plan so called the number to find out why it was taken from the account. The representative was rude and stated we should read the booklet where it states they don't do paper payment. When I stated we had been told different she stated "You must be having a bad day!" and hung up on me. I called again and asked for a manager who also stated that we could never have been told they would paper bill us. My husband was teaching school and when he came home he called the last person I spoke with and was told the same thing. He also was hung up on.
We have written a cancellation letter which will be sent out Certified, reported them to Texas BBB and talked with our bank to make sure no further payments would be sent to them. They have now been red flagged at our bank! Let me repeat: AT NO TIME WERE THEY GIVEN PERMISSION TO REMOVE MONEY FROM OUR BANK EXCEPT FOR THE INITIAL SIGN UP!
My husband was told that they would send our money back that we have paid so far but that remains to be seen. We have not gone to any of their providers. I did tell the person I talk to that I would think they would treat their members better as we were the money in their pockets.

All refunds were issued to the above members in a timely manner and in the form a paper check that was deposited and cashed by the above members. The above members were canceled immediately when their written request was received, as per their LEGAL TERMS AND CONDITIONS that all of our members receive with their membership packet and cards. (they have immediate access to the terms & conditions on our website upon enrolling) All billing information is recorded and Formulated has NEVER had a paper billing process. All matters in this situation have been resolved. This situation was a true misunderstanding. Our plan is designed to help those individuals who do not have dental coverage or insurance and we have grown nationwide by helping thousands of Americans across the country save 20-60% on their dental work. Again all refunds were issued in a timely manner, per written cancellation instructions.
