Jacqueline Coffer - APNR-FNP at Christian community health clinic on 9718 S. Halsted Chicago, Illinois 60628 has documented that she provided health health counseling tome regarding my health issues. She has NOT!
She has NEVER informed me of the kidney disease (stage 3) until my visit on [protected]! She never adjusted my medication for my hypertension even after I informed her that it wasn't helping me at my current dose.
Ms. Coffer states that I have't been compliant in taking my prescribed medications and that too is a LIE. "Valerie" the Clinical director was made aware of my encounter with this nurse months ago and "assured me it would be dealt with! It didn't happen obviously!
This nurse has been negligent in my care, negligent in informing me of things I made need to do to improve my health and she Definitely didn't tell me about a most crucial problem... My Kidney disease!

Desired outcome: There needs to be some kind of punitive action imposed on her for falsifying my medical information and the clinic needs to held accountable for allowing this to go on because I registered a complaint against her months ago
