I (Valerie Perkins) made a personal injury claim on January 11, 2024. I was told on the phone from Jimmy John rep to make a claim on line and someone will call me. I haven't
receive a call back from no one yet. it is January 18th, 2024.
I had a Doctor appt with my doctor, X-ray of my injury leg and MRI done.
I will start my Physical Therapy. Monday January 22, 2024. I would like to hear from your company on this personal injury to my leg. injury date was
January 9, 2024. I will be calling the Jimmy John on 5620 W Georgetown Rd. to report the incident to the manager there. I thought someone would had
contact me by now from Corp.

Jimmy John's - Wet floor, no rug on floor by door, no warning floor sign. Rug on shelf not on floor
While entering the restaurant on Tuesday January 9th, 2024 I slipped on the wet floor. In a lot of pain in my leg that night and all day next day Wednesday January 10th, 2024. called my doctor Thursday January 11th, 2024 was told to come in for x-ray. Called Jimmy John customer service Thursday January 11, 2024 before my appt with my doctor. customer service rep, told me to file a complaint on line. I told her I would after my appt with my doctor not able to put my weight on my left leg. what is the procedure in filing for personal injury.
Desired outcome: need to have procedure info, for health issues moving forward.
