Monarch Dental Subject: formal dispute of billing error and request for resolution

I am writing to formally address an unexpected billing issue that has recently come to my attention. On January 16, 2024, I received a statement indicating an outstanding balance of $487.00 for services rendered to my daughter, Naomi. This notice was surprising, given my understanding that all accounts related to Naomi's orthodontic treatments were settled in full, and she has not received any services from your establishment in recent months.

Upon receiving this bill, I promptly contacted your office on January 17, 2024, seeking clarification. A representative informed me that the charge was associated with a dental appointment dated January 7, 2022. I expressed my concern during this conversation, highlighting that my dental insurance coverage has been continuously active without lapse, and such services should have been fully covered.

Further investigation with my insurance provider, Moda Health, revealed that the billing discrepancy stemmed from incomplete claim information submitted by Monarch Dental. Moda Health advised that the necessary corrective action would involve Monarch Dental either providing the missing details or absolving the charges.

Despite multiple attempts to resolve this matter directly with your office on January 17, 19, 24, 30, and February 13, 2024, my efforts were met with unfulfilled promises of callbacks from management. It was not until February 20, 2024, that I was able to discuss the issue with a manager, who informed me of a claim resubmission that was ultimately denied due to filing beyond the permissible period.

This situation has placed me in an unjust position. My insurance representative, Amy, confirmed that Monarch Dental had initially submitted the claim on January 11, 2022, but failed to include essential billing address information. The claim was sent in as two separate claims. One had a claim with a picture of an x-ray for $130. The other was a 17-line-item claim. Both claims were for the January 7, 2022, appointment. Upon receipt, the insurance company notified Monarch that for the claim to be approved, a billing address location was required. After not receiving a timely response from a Monarch representative, Moda Health sent a subsequent notice on January 18, 2022, stating Monarch had 15 days to provide the required information. A Moda Health representative confirmed that Monarch representatives did not respond to the request for information within the 15-day window, or there after prior to December 19, 2023, and thus the claim was deemed closed/pending on February 8, 2022.

A subsequent attempt to correct this error from Monarch was made on December 19, 2023, well outside the standard one-year filing deadline, resulting in denial due to untimeliness on December 30, 2023.

It is my position that the responsibility for this oversight should not fall to me, the patient, especially considering the administrative nature of the error and the significant delay in claim submission by Monarch Dental. This lapse in administrative diligence has resulted in undue financial burden and stress.

Therefore, I respectfully request that Monarch Dental reviews this matter thoroughly, considering the outlined circumstances and the information provided by Moda Health. I urge you to take appropriate action to rectify this billing error by removing the unwarranted charges from my account.

Monarch Dental I am complaining about 30130 Monarch Dental, 3501 Towne Crossing Blvd, Mesquite, TX 972-270-6646TX 75150

I have been going to Monarch Dental at the address listed above for 6 years.

At the time, the staff provided a good service, and the dentists were professional and good treatment. On October the 30th, I had an appointment to have a crown installed, the stuff was different. They cleaned my teeth and signed some to install a crown the next day. The dentist installed a temporary and I was told it would last for three weeks until they get me permanent with three weeks. When I went home the temporary crown broke off into three pieces, when I talked to the manager, she mentioned some time it does happen. I told her that I was told by your staff the temporary will last three weeks. I went to the location; the manager was rude and unprofessional. I demanded a refund and cancel the permanent crown because I lost my trust. I mentioned to me to leave the premises. I rest my case in your hands.

Monarch Dental Monark Dental on FM.78 in San Antonio TX.

So I was given an appointment for 7:30 am on oct. 6 2023, but the only office didn't open until 8:30 am. I get in and they say the doctor is running late. He finally arrived about 9:45, he came to the room and just started to prepare for treatment, never say I apologize for being late nothing. So i'm already mad cause I was given this card that clearly say 7:30am. Then I have to sit and wait for you and you are late... So I told him how I felt and he just ignored my feelings about this type of treatment I have received by this staff... And because I complained, now I have to wait for a month 11/02 before I can get the service I need... I feel that this is unprofessional and I really would like to go somewhere else because if you don't care for your patient no better than this you don't deserve to get paid for such unprofessionalism... I have the appointment card to show proof. The staff here needs retraining as so does this doctor...

Monarch Dental Payment

It happened in October 2021. My tooth got cracked. They won’t take me until I pay in advance. Middle of my treatment dr notify me that she can’t do root canal, I’ll have to see other dr at different location after three days. When I went there they made me pay in advance also same amount. Last visit I went to 1st office for my cap, I notice they had special for full payment for 20% off. When I mentioned to office they apologized to not mentioning to me. I told them to fix the issue and refund my 20%. I wait until 4/2022 for them to take care issues. Finally I call credit card company to dispute. I should have dispute 2nd charge also, since they didn’t told me it’s going to separate, but i didn’t. Call corporate office several times, only respond i received is someone will call you in 72hrs. Now it’s been 2years and I received letter from collection office.

Monarch Dental Incorrect billing resulting in collections

I got a upper denture and lower partial denture Monarch Dental and within a year, the uppe denture cracked in half. I made an appt to get it repaired which the office said was possi but I was fitted for and given a new upper denture. When I first started getting billed, I contacted my insurance and was told that I was billed for a new denture. I contacted the office and talked to Lisa, the office manager. She told me not to worry about it because in the description, it stated that the dentures was too damaged to be repaired and they would appeal. I never heard back. We I finally stated getting hand written letters from the office, I went in. Lisa said since the insurance company didn't pay the full amount, her doctors didn't work for free and I would have to pay it otherwise the bills will be going to collections. No denture should crack within a year and don't threaten customers that if then don't pay or face collections. I talked to Benefeds and was informed that there is a certain portion of the bill that is supposed to be written of per the contract the government has with the dental plans and not charged to federal customers. Filing a complaint is my first step hoping it helps before I file a grievance with Department of Defense again about Monarch Dental. Thank you.

Desired outcome: 1. Stop sending hand written letters to my home. 2. Lisa made a promise to make things alright then when she couldn't or didn't keep her word, she kept silent and just started stalking me with letters and threats upsetting my peace.

Monarch Dental Billing

They have charged me different rates different from my insurance cost list. Had to fight for credit over 2 months back in oct 2022. Then another claim was sent out after I resolved my issue in dec 2022. That second claim gave me a credit of 64.89. staff member agreed to credit and took a partial payment on my next appointment in April 2023. Now they don't see credit. Now credit collection agency calling me for money. I called my insurance and they said they sent money in , February 2023. So money is no where to be found on their books.

Desired outcome: Better billing practices and update notes that they see credit so I won't be fighting after it was resolved.

Monarch Dental Monarch dental service

As of may 15 I have not heard back from monarch. I tried first to contact their hdq as well as requesting their callback through review processes. They have advised they would contact me on this site (as indicated below) and also via a bbb complaint that I filed. This is just their way of avoiding me and making themselves look like they care and want to resolve the issue. I will continue to post negative reviews until they keep their word and at least contact me. They have recently been acquired by smile brands inc. So I would be cautious about using any smile brands service groups.

On April 25, 2023 I arrived for my scheduled appointment to have a periodontal cleaning done. I had already discussed and agreed with the Manager, Robbie on the cost for this procedure. I was due to have xrays, Dr exam and a periodontal cleaning with fluoride. Upon examination I was told by the Manager, Robbie, that I needed a deeper cleaning that would cost anywhere from $550-$1100! I advised that I just wanted the normal cleaning that we had agreed on but Robbie refused the cleaning and advised that it was against ADA regulations to do such a cleaning if a deeper cleaning was necessary. I have since conferred with another Dentist and also the ADA and found that this is absolutely not true and that I should be able to have a normal periodontal cleaning performed if that is my desire. I have run into this situation before with this Dentist office trying to upsell me on unnecessary work which I have declined in the past. I refused the newer costly treatment and upon leaving I was forced to pay for xrays and an exam at a cost of $59 which I had to have done again by a new Dentist. I followed up in the review to Monarch and checked the box that I would like to be contacted regarding this matter. I never heard from them so I filed a complaint with the BBB. Monarch's response was they would contact me which they never did. The Dr-s and hygienist there have always been very professional however the Management is horribly rude and unethical to say the least.

I had a front tooth extracted in Dec 2022, which had come loose. It is now June 2023, and I still do not have the permanent tooth, due to office closures in the Dallas area, and general incompetence. I have a "partial bridge" which I am expected to use in the meantime, but am unable to eat with this useless contrivance. Four days before my latest appointment to have the final installment of the prosthetic tooth, I was notified that the tooth will not be ready due to problems with the shade/coloration, and by the way the office will no longer be open on Fridays. If I am ever lucky enough to receive this abominable dental implant, I plan to file a complaint of fraud to the Texas Attorney General.

Monarch Dental Dental cleaning visit

Monarch Dental Botched root canal

On November 28th, 2022, I was worked in for a severe toothache. After x rays I was told I needed a root canal and after being told the price the dentist proceeded, but it seemed to me, he was kind of in a hurry. While he was drilling, I kept telling them I was having a lot of pain, at which they kept telling me that wasn't possible. At some point while drilling I heard the dentist say darn and his assistant asked if he needed another file, at which he said yes. When finished I left still in pain, I retuned on December 16 for the crown which was too tight and he had problems taking it off to adjust it, where at some point he said it should be ok even with me telling him it was too tight, and I was still having pain. I had to return on December 22 because I could not stand the pain, the dentist told me was sorry for what I was going through and was sending me to an endodontist to see what was going on. Saw the endodontist on January 9th and was told there was a broken file in my teeth, and he was going to take care of it. I was told by the other dentist that I wasn't going to pay anything out of pocket. My tooth had to be pulled because the file couldn't be removed. This has caused me a lot of pain, trouble eating, sleeping and anxiety. I went to see the office manager to see if I could get my money refunded due to the dentist's negligence, but she said how would it look if they returned everyone's money four months after being treated, I told her I had been complaining since day one. She still hasn't sent in the paperwork even though she said she did. I paid 1,400.00

Desired outcome: my 1,400.00 refunded please

Monarch Dental 30320 Monarch Dental & Orthodontics 7650 S I35 Suite152 Corinth TX, 76210

I visited this location on 3/09/2023 for a 10:30am appointment. Unfortunately, I did not leave until 1:45 pm. My visit was for an ADA 09944 Occlusal Gd Hard Full 00150 Exam Comprehend Oral, 00220 Xray Periapical 1st 8, 00230 Xray Periapical ADL 9, 00230 Xray Periapical ADL 24, 00230 Xray Periapical 25 and 00274 Xray Periapical.

After they were finished, I went to the front desk and asked for the total amount due. I was advised the amount due was $385.00, which I paid in full. Image my surprise and disbelief, when I received a call from your business office on 03/17/2023, advising me I owed an additional, $150.00. I told the business office, that I will take care of the additional amount, that seemingly popped up out of nowhere, in April. Despite me telling your business office I would take care of the pop-up amount in April, I received, not one but, two calls on 03/27/2023. One from the business office asking for the $150.00, yet again and the other was about making an appointment to retrieve my Occlusal Gd Hrd Full and to see if it fit; that I made for 3/29/23. But to my disbelief and annoyance he (at this point) harassed me as well, asking for the $150.00. Keep in mind, this bill isn't even 30days late and I have been very patient and forthcoming with their gross mistake of not disclosing the mystery amount of this bill once services were rendered on 03/09/23 and I specifically asked how much I owed. So I advised him, as well, to please notate my file and I would take care of the $150.00 amount in April. On 03/29/2023, during my appointment, of course, you guessed it, they harassed me for this amount, yet again. And at this point, I believe the gentlemen deceived me in making the appointment to have me come to the office, to pick up my Occlusal Gd Hrd Full because, he refused to give it to me, unless I paid the $150.00 or set up payment arrangements. I believe he should of gave me this ultimatum over the phone when he made my appointment. After, that I got into a lengthy unproductive dispute with the associate that was sitting at the desk, insisting I was lying and was aware of the bill, when I left the office on 03/09/23. Unbelievably, she repeatedly, continued to pointlessly, falsely claim I was aware of the bill. After such unprofessionalism, I became so frustrated, I paid the entire $150.00 amount! I was so frustrated, I didn't go back to it fitted, for fear they would charge me some more trumped-up charges and not disclose them to me. I will never---, do business with this group of unprofessional, disrespectful people, at this location again!

Monarch Dental Had a tooth pulled

I went to the dental office at 1604 & Bandera in San Antonio, TX due to pain in one of my teeth. i met with Dr. Bates who said I had an infection and that the tooth was dead and should be pulled. My appointment to pull the tooth was scheduled for the next week at 9am. The office gave me a prescription for antibiotics. i arrived for my tooth extraction appointment 5 minutes early. At exactly 9am, the dentist that would pull the tooth walked in the door. Interesting. I waited about 30 minutes then was called to have x-rays taken. X-rays were taken the week before but, OK. I waited another 20-30 minutes to see the dentist. While I was waiting in the chair, I saw small black bugs crawling around near the floor baseboard. I brought this to someone”s attention. It seems, the dentist who would pull the tooth was an oral surgeon-Dr. Lang. why an oral surgeon? Dr. Bates told me it would be an easy tooth to pull since it was already dead. After the consult - Dr. Lang didn’t tell me anything new that Dr. Bates hadn’t said. Then I was told to go out to the waiting room so I could discuss the cost of the procedure. I asked why Dr. Bates couldn’t pull the tooth and was told he might not feel comfortable doing that. What is the purpose of Dr. Bates? To generate referrals to specialists toy bring in more money for Monarch? What a racket! I paid the $628 and had to wait again. Finally, at 10:55, I was called back to start the extraction. 5 seconds later the tooth was out. Basically, I had to wait 2 hours for a 5 second procedure. This is unacceptable. I will never go back to that office again. And, if this is how Monarch does business, I will think about not ever using this dental practice again!

Monarch Dental Business operation changes with no warning

Monarch dental scheduled me for an exam and cleaning. They, however, failed to tell me that they changed their rules where you get an examination and then you schedule for a second appointment. My time is too valuable to go twice and lose hundreds of dollars for not working. Why do they not give you this disclaimer over the phone so you can schedule both appointments on the same day? I am not happy now I have to takeoff another day just for a cleaning. After this visit, that is the last time I give them my business.

Desired outcome: Exam and cleaning on the spot when I scheduled it and was not the case.

Monarch Dental Refused Treatment

Dr. Moore at Monarch clinic at 1005 N. Collins Arlington Tx. Did a root canal on my lower jaw tooth, it was over extended and had to be pulled and bone replacement was put in. I was referred to surgeon Dr. Estekhari at the Monarch office on 5760 W. Pleasant Ridge Rd. Arlington, Tx. 3/9/22 for a consultation for bone pushing on the soft tissue of my lower left gum. He said he could take it off. I wanted to make sure it was bone and not the over extended material that was left in my gum. So I also spoke to Dr. Ingle. I called back 4/14/22 to make an appointment because its hard to lay on left side of my face sleeping because its pushing against the soft tissue in my gum. I talked to Rachel in the Pleasant Ridge office to make an appointment with Dr. Estekhari. Rachel proceeded to tell me Dr. Estekhari is refusing to treat me. Rachel said I needed to get with my insurance and find another surgeon not with Monarch. I was shocked. So if I'm understand her if your dentist have a problem Monarch just denies treatment to fix the problem. We have been patients of Monarch Dental for 5 years. I have never had to file a compliant until now. Dr. Moore made mistakes but he always took care of them. I need Monarch Dental to refer me to a surgeon so I can get this taken care of. Monarch Dental Refund and service

I've always had a great experience with Monarch Dental in Little Rock Arkansas but here in Garland Tx is the worst. After visiting the Monarch on BELTLINE rd in. Garland TX, I'll never trust Monarch with my or my families needs. Over pricing I assume is this Monarch way. The Doctor wouldn't subscribe pain meds unless money was paid. I was in extreme pain and was required to make a 200 payment down towards an extraction. After request the Doctor stated at 8:45 am that she would prescribe pain med (ibuprofen) at about 12:30 noon I went to Walmart and was advised nothing had been sent. Contacted Monarch in which the rep advised that the Doctor stated that he would send it now. I requested a refund and was advised that the payment was voided and the funds would be placed back into my account. The funds are not there as of this morning. I have filed a BBB complaint and was able to have my tooth removed the same down on half the price. I want my full refund of 200.00 please help.

Monarch Dental Did not file claim with insurance company

Description: Prior to appt. gave the office my insurance information was told the insurance would pay, made appt for 11/7/19 - appt for a consultation, brought xrays with me. Dr. spent less than 5 minutes to inform me everything was good. Went to front desk to pay balance of fee insurance did not cover, was told insurance would pay in full. Started receiving bills, never rec'd an EOB from ins.co. Called ins to find out why they did not pay, they never received the claim. Rec'd another bill on 6/8/20 talked to Lyonel on 7/22/20, he would file ins. Left a msg with billing on 8/17-816 am and 11:02 am, again on 8/25/20 1:45 no returned call. By this time, went to a collection agency I spoke with Maureen and Paul Kleppe on 8/25/20, told them the issues, they sent it back to Monarch to send in claim. Spoke to Erica on 6/16/21 was told she would send in claim since one had not been sent, but no follow up from her. Rec'd a bill from a collection agency on 8/4/21, called billing at Monarch on 8/10/21 to tell them I was awaiting a call from Erica of the follow up on the ins. Monarch was told they sent it to collections and my call needed to go to them now. Called collection (RGS) spoke to Nehemiah Palmer (10:08 am) told me I needed to settle or they could send back to Monarch to file the claim with insurance, however, I was told this would hit my credit score in 45 days. I ended up settling the claim. My only issue was all I wanted them to do is file the claim with my insurance, this never happened. I gave them my insurance info every time I spoke to them. I would have paid in full if the billing department would have just filed the insurance claim and communicated with me of the progress. We pay a high price for insurance and we should have the right to have insurance filed especially when the billing department claimed they would do it.

Desired outcome: File with my dental insurance company

Monarch Dental Office management / scheduling

I'm a new patient, so I had my original consult and a follow-up appointment for dental work. The short version is this office has no office manager, so appointments, waiting and overall time spent there is absurd.
Ex 1: First visit, I had all paperwork filled out in advance to save time. I still waited in the lobby for 45 minutes before being taken back. After that, the visit ran long and I was late for work. Keep in mind this was for an initial consult, x-rays and teeth cleaning. They were so behind schedule, I opted not to wait another hour for teeth cleaning and left.
Ex 2: I scheduled my follow-up visit and was told 90 minutes. The visit took 5 hours. Apparently they overbooked and are understaffed. They would work on me, leave for 10-15 minutes, do some more, leave me again, etc. I was late to work again and docked pay because they lied about the time and took way too long.

The Pros: Staff is friendly, dentists do a great job.
The Cons: They don't have a local office manager, they're scheduling stinks, they're understaffed and can offer nothing but excuses when they go over their allotted time.

I got a manager (Sandra Contreras) on the phone and received an apology, but they refused to reduce the bill for this screw up. I will not be returning to this dentist office. Just not worth the hassle and wasted time.

Monarch Dental The reason I went there was not addressed.

I went to Monarch to have 2 crumbling teeth extracted in 2014 and to this day those 2 teeth still need to be extracted. Instead of taking care of the work that I went there for they used my insurance to do a deep cleaning and on top of that charged me an extra $500.00, I had to take out a loan and pay it back on a monthly schedule. A female dentist came in and recommended that I have a bunch of teeth pulled and there was nothing wrong with those teeth. I left the dental office in Corinth, Texas, and never returned, that dentist should not be allowed to practice. Prior to that incident, I had gone to the Monarch in Dallas at the Casa Linda location and was treated very.
I would never go back to the location in Corinth, Texas, nor would I recommend it. A friend of mine took her teenage daughter there and walked out when they told her the girl needed a deep cleaning. She took her daughter and her money elsewhere. Monarch seems to be a crooked company with a bunch of crooks working for them.

Monarch Dental Insurance claim pre authorization not done

Monarch dental in Red Oak, Texas told me that I had a lot of dental work that needed to be done. I gave them my insurance information whici is Humana. They came back and told me that my insurance would pay for $11, 000 after getting a preauthorization.
Now 5 months later they are charging me for work that was supposed to be covered in the claims send to Humana. when I call them they told me that they never got a pre-authorization from Human.
Humana also said they recerived a pre authorization request. so how were they able to give me a total that would be paid?
Now they are threatening my credit with the money they say I owe.
the money was not paid because they did not do the pre-authorization.

I have more dental work to be done but I do not trust them to send claims correctly or even t all. they want me to pay it and I am a senior who cannot afford this

Monarch Dental $1,461.12 refund for service never performed.

I requested refund at the Mansfield, TX office on 21/12/2020 and the billing clerk, Christian stated that she would expedite the refund to me. I called the office back on 2/25/2020 to discover that she had done nothing, but she stated that she would make sure that the refund would get processed to the corporate office as soon as possible. I called on 3/16/2020 and Christian stated that my refund had been processed and mistakenly mailed out to the wrong address on 3/5/2020. I asked her to credit my refund to my credit card so that I would not have to wait another 4 weeks, but she said she could not do that until she receive instructions from the corporate office. Christian stated that she would email the corporate office for instructions. I called the office on 3/17/2020 and 3/18/2020 and was told by an office clerk, Jennifer that Christian will be out of office until Monday, 3/23/2020 and there was no one in the office to help me. I am very, very frustrated. My wife and have been Monarch patients for Ten years. I need someone help me to receive my full refund of $1, 461.12.

Monarch Dental miscalculated billed /credit ran for no reason

Hello, To whom this may concern.
I've been going to this location for awhile. Billing clerk switch and issues began.
First issue- my credit checked for no reason, if my file would have been looked at properly it wouldn't have happened.
Second issue- the clerk miscalculated a Service not taking into account the service was for a back tooth and then called me back about the bill 4 months later to tell me I had a late bill. I knew nothing about.
I feel as those with all the mistakes I should not have to pay this bill of $114.00 and I absolutely don't want to go back to Monarch because of it.
I've called and left a message with the monarch location and they didn't return my call. It's been a week since the message was left.
