Aspen Dental review: Hipaa violation for medical records

I requested my full dental records going back to 2021 in September 12th 2023 through email to the office manager Nikki Bloom she sent me a release form to fill out and send back which I stated I wanted paper copies MAILED to me. She was very helpful at first told me it should only take a couple weeks, then she told me she put my old address on accident even though I wrote my current on the release form. A month goes by I request it again, she informs me that she had it expedited so it should be here very soon. Another month goes by she still doesn’t know why, by December I demand them, she sends me a 55pg. Email of screenshots “this is what I can view to hold u over” I immediately noticed 3 forged signatures, $495 visa payment that I never made with no last 4 digits. Aspen, when I questioned it she said she wasn’t in the office and couldn’t access my financial as far as the records she said I can review the 55pgs never responded again after that. I have sent her several emails and left messages since with absolutely no response or reason. But they just sent a 55$ bill to my correct address so they clearly have the right address, It has now been 6 months. I’m requesting my records because they botched a temp bridge in 2021 that I’M STILL WEARING. Have given them more than enough time to correct this before taking action.
