Arizona Medical Board negligence, documentation not correct, no medication, and lack of professionalism to start

Been having continuing problems with PCP, Dr Christopher Hiler, and staff at Desert Bloom Family Medicine and other office at 43rd An and Thomas. Have tried to talk to medical director, Dr Hiler, but not his department and handed me off to office manager. The following are what they haven't done.
~ Have contacted numerous times by myself and my pharmacy regarding a refill on a medication. Still have not received to date.
~ Documentation is all a lie. Reason for visit is not correct at all. All visits pertain to my severe chronic migraines.
~ Insurance company never received pre authorization for new migraine medication that seems to be working. Still has not received pre authorization to date.
~ Bathrooms have poopy toilet in trash and it's gross. I will not even leave use due to dirtiness of both bathrooms. What species does that... Mexicans!
~ Also, over charged for MMM card, $200. Office visit is cheaper. Call and ask if it includes they send in application, will state yes, however that is also a lie. I wasn't informed correctly and now have to do all over again.
How can any practice be so wrong in so many areas? And still practice?

I went to dr. Mcconnell on february 14, 2019, prior to that I had brought in a medical clearance from my primary since the office denied ever getting receiving it. Arizona Medical Board incompetent doctor and staff

I take 6 prescribed medications. I am on Medicare and also have Humana, My RXs are to be Faxed to the Pharmacy Dept of Humana. I have a blood test every 3 months due to Diabetes. I constantly have problem with Dr. Wood's staff faxing prescriptions to Humana. I have even had Humana's Pharmacist call me and asked me "what is their problem?" They never return a FAX request to contact Humana. I would say that they never return phone calls but they do NOT have an answering machine! I have never seen anything like it! on Friday they work 8 am to 11 am. Why bother being there at all? They leave every other day at 4 pm and are closed for lunch between 11 am to 12:30 pm! That is crazy! Especially when no-one can leave a message. I have written letters to the Dr. several times about his inept staff. Even hand-delivered them and pushed under door because they were closed...of course!

I went without meds for 1 or 2 weeks but this last time (about Nov 2018) I was without my meds for an entire MONTH! I take Metformin and Paxil and Thyroxine (3 of my meds) and you don't just stop taking these meds COLD TURKEY! Dr. Woo would never call me...every time I called his staff to re-FAX RXs they always say they did and it was so-and-so's fault. Or Humana's fault! IT IS ALWAYS THEIR FAULT!

I moved and found a doctor closer to me (blessing in disguise) and called them and gave them his address, name, phone number etc and told them to send my medical records to him or I will pick them up. I did this 30 days before my first appt with my new doctor. They never sent my records to him! I went into office and they said I had to fill out and sign a form. This was first time I heard this. I signed it and the Nurse (Karen) told me to get the f**k out and don't ever come back.

I WANT MY MEDICAL RECORDS EITHER SENT TO MY NEW DOCTOR OR TO MYSELF IMMEDIATELY! Dr. Woo needs to retire. None of them care about their patients!

Arizona Medical Board medical billing/records

I have requested my medical records and an itemized billing statement 4-5 times now by phone and email-to no avail. On top of that, I have paid my bill in full and the company continues to bill me for what I do not know.

I have it in writing from them "paid in full". The bill is paid and has been for months.

I want a copy of my records and an itemized billing statement.

Arizona Medical Board incorrect sized implant used

I had a total knee replacement in April of 2018 performed by Dr. Lance Winter. Before I went back for surgery, Dr. Winter told my husband and I that the procedure would take approximately 90 minutes. Mine took 3 hours. When my husband asked what took so long he was told, everything went fine. The truth was far from fine. I ended up with "drop foot", blood clots that went to my lungs, 11 days in the hospital (with a lot of $ out of my pocket), and a lot of pain. After 7 months of very little progress, I went to another surgeon for a second opinion. The second opinion surgeon took x-rays and I found out the implant that was used was 1 to 2 sizes too large for my knee. The metal pieces are hanging over the bone which is causing problems with my tendons, ligaments, and nerves. My muscles are working overtime to keep my knee stable, therefore, I deal with swelling issues on a daily basis. My option is to do it all over again or learn to live with the pain. I am wondering who will pay for the second surgery and subsequent therapy.

Arizona Medical Board refusal of follow up treatment. surgery apparently done incorrectly. refusal to treat after referrals from provider.

My son Scott sustained broken bones in his right leg and ankle on 7/21/18. Dr. Michael Billhymer, Sonoran Orthopaedic Trauma Surgeons, did surgery 7/22/18 with a metal plate and 6 screws to secure the bones. Followup xrays showed the bones not healing together and (1) also showed the bones were not placed together when secured. The bones are not mending together.
(2) Scott obtained AHCCCS Ins. following the accident. A 3rd followup exam SET BY THE DR. for Oct. 30 was cancelled by the Dr. as he said it was outside of the 90 day treatment window. ( 10/21/18 ) Scott would have to go back to his family Dr., get a referral, and get the Ins. Co. OK for Dr Billhymer to continue treating. (3) Scott now has a new Dr. who is UPSET that A-Treatment was refused B- AHCCCS patient was refused C- Surgery with plate and screws was not done properly. (4) Scott is now looking at another, CORRECTIVE, surgery.

Arizona Medical Board aftercare at the va

HAD kidney stone procedure for right side at VA. NO aftercare provided, no pain meds or antibiotics. This time left side going to be done. Asked to stay overnight to control pain symptoms and was told no. This is unacceptable since I ended up in ER After last surgery.. even explained to them (Dr Joesph Westler) that several other out patient procedures turned into ER visits immediately afterwards. Please help stop VA policy of refusing aftercare with my past history of complications .

Arizona Medical Board not providing care/negligence

We have been going to Dr. David Izenburg for more than 2 years, my wife and I have diabetes and my wife is recovering from endocarcimona a lung cancer, I have RA, Tore Menicus in right knee, and very bad bulging discs in lower back. In the 2 years we have been seeing Dr. Izenburg my knee is still tore back is still in bad shape, but the reall negligence is when he out of the blue dropped our insurance and never notified us about it, so we are stuck without Dr. and not refills on our meds, we are at this time trying to get another Dr. with insurance they will except. I've been getting back injections that don't work and with insurance I have now doesn't give me the proper references that is needed to get me fixed up to even function. The office took our blood on one day and next day called us to tell us they didn't draw enough, so our results were inconclusive, but didn't draw on that day and never called us again. I was very sick one day and went in and left specimen in sterile cup, and went in 2 days later so I may get antibiotics but they never get specimen. Not very good with patients and doesn't follow through with anything.

Arizona Medical Board my dr. closed his practice and never notified me or said where my records are. the dr. is gary smith, 7400 s. power rd., gilbert, az 85297.

I was only made aware because a PA of his Tracy Stevenson, informed me she left his practice and took my records. She said to make an appt. at Desert Grove Med. I needed drug refills. I made an appt. there. I was 5 mins late. They refused to see me. I said give me the appt who took mine. She could not comprehend that. I asked for my records. I got 4 pieces of paper. I was told WHEN I find a Dr. request them and I will get. I still don't know where my records are.

Arizona Medical Board physician with an az temp license seeing medicare patients without a medicare provider id number.

Good morning,

I am very concerned about flagrant and abusive attitude of this physician who continues to see Medicare patients without a Medicare provider ID.

Dr. Catherine Torres, working with a Temp License from Az as of Dec 2017, has no Medicare Provider ID number, no permanent AZ license has been seeing multiple Medicare patients since October 2017 to the present.
This is huge cause for concern as physicians are held to a higher ethical - medical legal standard.
This is a red flag and huge cause of concern. Her bold and flagrant behaviors flaunt Medicare guidelines, and medical legal recommendations and guidelines.
She is seeing multiple Medicare patients without a Medicare Provider ID number and she has access to all of these Medicare Insurance Cards with Medicare Insurance number (Social Security Number) which viewable to providers with current Athena electronic health record system.

Concerning that she has no AZ DEA number and has prescribed opioids using her Oklahoma DEA certificate.

Obviously my concern is to protect patients from individuals who abuse the system and commit fraud. Please can you help here.

Arizona Medical Board neglect/death of my mother

My mother was admitted to mountain vista medical center, after her surgeon dr.shennib had advised her to go to er with complaints therefore he can perform surgery, even though he was aware her insurance was not good for his hospital, he performed stent surgery which was supposed to be 3 hours and lasted 11hours, he was well aware of her condition, she was unable to breathe on her own after surgery, she was incubated for 2 weeks, she was then transfeered to abrazo medical center, because insurance, she had traich put in by dr.alrabadi whom was very helpful and was doing better, dr. shennib was not very helpful, he advised me to put her in hospice she was 58, she then was transferred to st joseph select specialty where she was getting physical therapy and we were learning how to take care of traich, she was to be released but on sept 16 something went wrong they restrained her in middle of night, which had never happened in the 40 days she was hospitalized, and passed away, she wanted me to talk to someone about dr.shennib, i believe st joseph needs to explain also.i have all medical records and pictures of bed sores.

Arizona Medical Board unethical behavior

I know a Registered Nurse by the name of Georgina Marie Carroll, born 04/1971. She works for Gallus Detox centers. I am reporting her unethical and illegal behavior. She has been using Doctor Patrick J Gallus's name to prescribe friends and family prescription medications. I know a registered nurse is supposed to be smart. But she prescribed me a medication without knowing what I was allergic to. I was dating her son at the time, and I didn't know it was illegal or what she was doing was illegal. Her son Anthony Conrad born 07/04/1989 told me she does it for him all the time. I assumed ok I'll take it. But now knowing this behavior is inappropriate and illegal I felt I need to report it to someone. I have never been to a treatment facility for drug use in my life. I have attached proof of this. If you have to call to verify I have never been in this facility you can. You can look up all her kids and family by name and date of birth at pharmacy's and see she does it for everyone. But again on my attached photo, you will see Patrick Gallus as the doctor. I have never been inside his building or met this doctor. Georgina Carroll works for him and prescribes medication using his name when calling them into pharmacies.

Arizona Medical Board diagnosis of my epilepsy

I have had epilepsy all my life with all doctors to prove from epstien to saunder burns to thomas thacker and i am appualed in ur practices. They decoded to say you dont have epilpsy per carolyn of terros and also thr physchologist of mesa saying u have bipolar. No i have eeg ekg proof that it is completly invalid and have had enough of medical abuse. In addition I have been told with epilepsy that I have to be gravely disabled no Im a single mother ran my home cooked cleaned and even living with my parents paid wat i could the min 100 per month and have also a letter that was for des to prove i was builying and taking care of me and my daughter with her signature. Perjury. In addition i have facebook proof showing i never said any of the fabricated false accusations that was put on the petition. An overall I am requesting to have all illigitament allegations removed for fabricating false testifing and organized crime of social workers. Such as kim rankin abusing in a text saying u take depakote for bipolar not epilepsy that is not a drug for epilepsy. In proven text also. I have to take this also up with navajo my clan the congress and others for civilian rights attack and disability targeting and domestic violence manipulation of a life.

Arizona Medical Board granit creek health and rehabilitaion center (prescott, az)

Background information.

On Jan 12, 2018, I Melissa Dubaj took Steve Dubaj to the Yavapai Regional Medical Center ( Prescott, AZ). Steve Dubaj was admitted to the hospital for having trouble breathing, high blood pressure and low oxygen levels, after running some test the doctors discovered Steve Dubaj cancerous tumor on his lung had gotten bigger and was starting to cut off his oxygen. So on Jan 15, 2018 Steve Dubaj appointed me to be his medical power of attorney incase he got the point where he couldn't make decisions on his own. And that's also when Steve Dubaj was put on Hospice Family Care . People from Hospice Family Care and the doctor discussed with Steve about transferring to a health rehabilitation to try and get stabilize to possibly go home. On Jan 17, 2018 Steve was transferred to the Granite Creek Health and Rehabilitation Center.

My Complaint

After going through Steve Dubaj medical records from Granite Creek Health and Rehabilitation Center, I discovered in his medical records they had Steve Dubaj sign off to start taking a medication called Lorazepam for anxiety on Jan 17, 2018.The attending Physician was DR. Kruprnrck. MY complaint is Steve Dubaj was obviously heavy medicated when he signed off for the Lorazepam on Jan 17, 2018, because his signature isn't clear and doesn't look like Steve Dubaj's signature. Steve Dubaj shouldn't have been allowed to sign off for the Lorazepam when heavily medicated, they should have got me, (Melissa Dubaj) permission to start to him on Lorazepam for anxiety. The hand writing isn't clear for LPN who signed off on that record, but it looks the initials LCL or ICL .

After receiving Steve Dubaj medical records from Granite Creeks Health and Rehabilitation Center on April 22, 2018. I discovered a majore problem. Granite Creek Health and Rehabilitation Center had a record saying on Jan 19th I (Melissa Dubaj) gave them permission verbally to start giving Steve Dubaj a drug called Terminalttaldol for agitation and I know for a fact on Jan 19, 2018. I did NOT give them permission to give Steve Dubaj anything for agitation.

Also Jan 19, 2018 Melina Mckg. RN wrote a prescription at Melissa Dubaj request for the staff to check Steve Dubaj blood sugar before each meal or if Steve Dubaj doesn't eat, because of his diabetes. And again in Granite Creek Medical Health and Rehabilitation Center records said it was checked on Jan 19, 2018 and Jan 2018 by someone that goes by the initials FNTW, when making documentation of what care Steve Dubaj had for that day, the problem is Every Nurse that I spoke to from Hospice Family Home Care and the nurses from Granite Creek Health and Rehabilitation Center told me over and over on Jan 21, 2018 that they did NOT check Steve Dubaj blood sugar at all and they weren't going to check Steve Dubaj blood sugar, because it wouldn't change the outcome of his health. Either someone falsified there records or I was lied to.

Then also in Granite Creek Health and Rehabilitation Center records they have a record dated Jan 20, 2018, that an order was given by phone for them no to care for Steve Dubaj's diabetes anymore, but it Does Not say who gave the order to Discontinue Steve Dubaj diabetic care.

I don't have a way to scan the decimations in my computer, but I have hard copies of all them that were mentioned in my complaint.

I just want someone to pay for providing really horrible care to Steve Dubaj when he was there from Jan 17, 2018- Jan 21, 2018 when he passed away and make sure no one else will go through that horrible care ever again.

on oct 27-2017 dr steven correa cardioligst put a stint in me not needed had several complications from this told him I had worst chest pain never been out for one procedure all of the sudden six and half hours later woke up girlfriend said correa put a stint in went on for three days still worst chest pain told dr correa this he brought me back to the cath lab said nothi9ng found chest pain x3 went to banner hospital phonix Arizona dr peter maki foun a blood clot on the stint got another stint got back to wrmc asked dr mirza outside why dr. coreea had a problem communicating dr mirza another cardiologist in bullhead city Arizona told me who could not talk to me at the hospital but dr correa messed up I asked him what did he mean by that in the emergency room the hospital administration showed up not announced and dr mirza was trying to get me not to sue never caused a problem dr mirza told me that dr correa asked him to look at my xrays I told dr mirza he had to consult me I was the patient and then met me and pam at Walmart told us to ahead with a lawsuit that it was valid I told wrmc hospital ceo tom selarno that information I told you and said what do I want out of this I told him that medical records clerk told me my file came across her desk three times for legal issues I informed him also dr correa left me to die on the table she said he nicked somebodys aretery the corporate office got sued by the federal gomverment for medicare and Medicaid fraud 891 millon they paid the federal government for overcharging doing unnesary evasive procedures and dr mirza got but on probation in California and Arizona mty name is paul Broxton

this doctor and hospital needs to be monitor that nothing ever happens to a patient like myself are anyone dr Michael tribemen er doctor cussed me for a son of a [censored] I have this document tated the hospital let go of dr steven correa and wrmc tried to erase my marc record this hospital and dr are responsible because dr steven correa and wrmc shall be accountable for what the done I have several outstanding witness that will come forward the messed my heart up and they shall be accountable dr correa should be held for attempted murder

Arizona Medical Board arrowhead pediatrics

My granddaughter was a patient at Arrowhead Pediatrics for many years.

She has turned 18 and has started to see another doctor. The office abruptly closed and we have no idea how to get her file or a copy of it to submit to her new doctor. She is having issues and we need the get this information.

Please let me know what we can do.

Arizona Medical Board dr. fernando decastro

My name is Elese Weaver & my husband's name is Buster Weaver. My husband is 70 years old & in very poor health, he was a patient of Dr. DeCastro's for over 15 years. At the beginning of February he went to the hospital because he could no longer urinate on his own & had to be catherized & still has the catheter in. While I was at the urologist's office I spoke to Carla at Dr. DeCastro's office & told her I wanted to cancel our February appointment because we had to be at the urologist's office. She never canceled that appointment & as a result we were discharged from the Dr.'s practice without notification. When I called 2 weeks later I was informed that my husband was no longer a patient because he had misses 8 appointment's in 15 - 20 years. I was told that if he needed any of his prescriptions refilled they would do that. To make a long story short I called in the prescriptions and they never authorized them, then I called them directly and they refuse to refill his prescriptions because it's been a month since he was discharged from the practice, without notice. I informed them that I have tried to get his 2 prescriptions filled for over 3 weeks to no avail. Now my husband is without his anti seziure meds or his metoprol & they refuse to help. I have an appointment on Monday with a new Dr. It would just be nice if the doctor would live up to his word after 15 - 20 years of my husband's patronage.

Arizona Medical Board eugene trowers and banner umc

I had a referral from my PCP to see a gastroenterology MD. Recently my insurance changed and I work very close to the Keno Hospital. I had my referral to there. It took me calling for several weeks to just confirm they had received me referral and only after I faxed it myself. I had been throwing up daily for nearly 6 months. When I scheduled my appointment I gave them my number and asked that they call me if someone canceled to get in earlier. I had nearly a 3 month wait.
When I arrived for my appointment I got there early to do paperwork as asked. I never completed any paperwork. Prior to my appointment I had all pertinent records faxed to the office. I waited over 2 hours to get called back and then waited longer for the doctor. When I saw Dr. Trowers I complained about my wait. His response was "I was lucky, some people waited longer and some people waited over six months to see him." I did not feel lucky. After finishing he told me he was going to get me scheduled for an endoscopy (he would be doing it) and then wanted to see me in 2 months. I waited over 20 minutes to check out and witnessed 2 employees verbally fighting over who had to check me out. The one who lost was very unprofessional and mad at me for inconveniencing her. I told her Dr. Trowers wanted to see me in 2 months her response was "good luck with that." I was told he was scheduled out further and I would need to call one of his assistants. Dr. Trowers had also stated that I needed to let this person know I was being scheduled for an endoscopy. When I told her she said I don't schedule those. I said ok, but he wanted me to let you know I was having this, she said you didn't hear me, I don't schedule those. I saw another person walking in the hallway and asked to speak with the office manager or someone in charge. The person stated that it was after 5pm and management usually leaves prior to 4pm. I got a business card for the manager, left messages the next day, that did not get returned and repeated calls went to a mailbox that gave me a message it was full. My persistent calling did get me a n advocate in his office who helped me.
After 2 weeks and no calls from the hospital GI lab (also unable to contact them, unanswered phone and unreturned messages) I was told they were waited for auth. from my insurance. A call to my insurance rep. informed me that no pre-auth. was required for this procedure. I called my advocate and left a message. While I never heard back from her, I did get a call that afternoon from the GI lab who could get me in on Thursday 9/28 or I would have to wait until Nov. (this was on a Tuesday). As uncomfortable as I was I took the appointment.
I received prep calls from an RN. I showed up early as directed from my appointment. The clerical staff told me I needed to pay over $900, which I did. In her office I pulled a bottle of water and put a teaspoon of water in my mouth to wet my mouth and spit it out. She immediately told me they were not going to be able to do my procedure because I had drank water. I thought this was an odd decision for a non-clinical person to make and requested to speak with the MD or a clinical person. I never did until 3 hours later. The clerical person came back and said fortunate for me they would get me in 2 hours later, I still insisted on speaking to a clinical person as my stomach had not see the water that was in my mouth but my words went unheard.
Several hours after my scheduled procedure I was taken back and the procedure was done.
Afterward Dr. Trower stated he needed to speak with me and my wife immediately. We were taken to a room and told I HAD esophageal cancer or precancerous cells. I needed to see him in a month, was prescribed increased proton pump inhibitors and told that official pathology would be available in 5 days but "no news was good news" and I would see him in a month. Over a week went by and I had heard nothing. My previous experience with all of this health care organization had left me less that confident in them. Calls to Dr. Trowers office and my contact person there who had previously helped me went unreturned. attempted to contact medical records and get my pathology reports. Calls to medical record' went unreturned. Calling an operator and asked to speak to a human in that department meant being placed on hold for a long time and then being disconnected. I finally insisted when speaking to an operator to speak with a patient care advocate. this person informed me that a I could not get my medical records not even through a portal. She said it was a perfect storm and that they were changing computer systems. In the meantime I cannot sleep at night, I have a 6 and 7 year old children at home who need their daddy for a long time. The person stated I should contact Dr. Trowers office for this information. She attempted to call them as well. She called me back and told me she also was unable to speak to someone there. But she would keep trying.
Two days latter I was contacted by Jeff at Dr. Trowers who said all of the pathology was negative. I asked him to send them the pathology, I have yet to receive it. I asked why in the preliminary report (that I have with the pictures) Dr. Trowers states I Have precancerous IE Barrett's esophagus. He was to speak with Dr. Trowers and get back to me as well as set my next appointment, that was over a month ago and I have yet to hear back.
I feel I need to re-due this whole process with a more competent health care system. I have no confidence in any of the services I received. And what about the bright salmon colored tissue Dr. Trowers saw and showed me pictures of that he was sure was Barrett's?
After sharing my experience with my PCP he wants me to repeat somewhere else. I need to wait until I can afford it again.
On tome of everything yesterday I received a bill from Banner University Medical Center and they do not show that I paid the $900 plus and want another $456.08. Good thing I have my receipt from 09/28/2017
