I had a major incident last evening after taking my vehicle from Midas shop located at 34 Ave & 99 street in Edmonton, AB. The front driver wheel almost came off when I hardly drove for couple minutes on 34 Ave & Calgary trail, brought the vehicle back to shop, it was found that 4 off 6 lug nuts on front driver side wheel were completely missing and remaining too were barely engaged on studs. Shop staff response was very disappointing, they straight way stated they didn’t remove the wheel. Unless I see the full footage of the service done in your shop, I have all reasons to believe that shop tech left the wheel with loose lug nuts. Part of my drive last night was on hiway, it could have been a very serious accident had I been driving on hiway for little longer at 100km/hr or so speed.
I need Midas to thoroughly investigate this incident, share complete findings with me including full video footage of your shop indoor cams, and if the shop was at fault then I demand thorough inspection of my vehicle and complete repairs of all possible damages to axle system as well wheel, or any other damage to the vehicle which this incident might have caused, at no cost to me. Note that I’m already noticing lots of vibration on my steering wheel and as a caution checked wheel once vehicle on jack and found out lots of “play” on front driver side wheel bearing, which definitely because of the incident.
This also caused significant inconvenience & concern to me & my family. I’m still in shock considering the fact that this incident could have lead to very serious accident, if I don’t receive proper response from Midas management in very timely manner, I reserve my right to take this incident to next level to the relevant government’s vehicle safety entities at provincial levels.

Claimed loss: Reimbursement of replacement costs for wheel bearings, alloy wheels, and new lug nuts of same type on all wheels
Desired outcome: Above mentioned reimbursement at the rate which Midas charge to their customers
