Pearl Vision review: Failure to provide receipt for prescription sunglasses for FSA approval

I purchased a pair of prescription sunglasses from Peral Vision located at 3000 Danville Blvd, Alamo, CA 94507. When I want back to get a more detailed receipt, the business was closed with no forwarding address. I have tried reaching out to Peal Vision but cannot find any contact information.
