Cartersville Medical Center Doctor patient confidentiality

12/7/2021
Carterville Medical Center,
I was in the ER on 11.20.21 and 11/29.21 Account number V09000301923, I'm afraid to share even basic information with your company. I mailed a similar copy of this form to my lawyer Farrar and Farrar, Georgia board of Health Care Workforce, etc.
I came to your office for a problem in both my eyes. I had stated that I was under duress. I could not see the forms I was signing. I told the receptionist that a different company referred me. You had asked if this was a workers compensation issue and I said, yes, this did happen at work but it happened 6 days before my trip to Cartersville. my lawyers Farrar and Farrar did not want me to go through the workers compensation process. I stated that I would pay for the visit myself. I even put a $20 payment down to see the doctor.
Before I saw the doctor, you had many forms for me to fill out. Without my eyesight, I did not and or understand what I was signing. The first form was consent to treat which the copy I reviewed was blank. I believe I signed that paper. Next was asked about information from my employer. I was coerced into signing a medical release form under the assumption that I would not be able to be seen by a doctor. The receptionist said this was normal protocol and that I could change this decision at any time. After I saw Dr. Johnathan Gore and was treated, I immediately came back to the desk to delete this medical approval. I was not allowed to do so. They tried showing me that I had 90 after treatment to do so. Obviously, I could not read that and it was not explained to me. When I left I asked for copies of the records. Your office did not have them ready. When I returned on the 29th that month a copy of a drug screen was included in the paperwork. Since I paid for the ER myself, I would remember receiving a drug test. Looking over it my social security number is wrong and it was stated a woman took this test.
HIPPA violations are a serious matter especially when patients are mixed up. Now not being under as much duress I hope that you did not send medical records to anyone but me. It seems I was deceived in my state of mind, and I had no neutral party helping me with these documents. I wish to be the only person to see my records and I am forwarding this message to my attorney because I was under the impression that the work injury took place 6 days before and I was going to pay for this visit,

My Wishes are simple. Immediately do not give any of my protected health care information to anyone but myself. Especially 11/20/21 and 11/29/21. I need to know whether you sent any of my health care reports! to whom and when; I asked for a copy to release information to my PCP and Attorney but I have not willing done so. I stated to the receptionists that I was in pain, under duress and really felt like they had coerced to sign something that I could not possibly see. The only thing verbally agreed with was the right to see the doctor. Nothing else.
If there was information that had leaked I will have proof it was you! Criminal cases are without a reasonable doubt; But in civil court it rewards the party that is 51% likely.

Desired outcome: If a medical release form was signed i want it destroyed immediately. And If you had relesed my medical records; youll have to think something. say it was a female with same name.

Cartersville Medical Center Patricia Chu Mulder

My name is Alyssa Scoggins. I went to the ER last Friday due to very bloody stool, dizziness, blurred vision, headache, stomach ache, and fatigue. I tried making an appointment with wellstar urgent care and was blocked and told to go to the ER. My experience in the ER was the WORSE I have ever been treated. I would not take my dog to see Patricia Mulder or Cartersville medical because of the way we were treated. First walking in I explained to her my symptoms and told her I had a picture of my stool and showed her it explaining how dark little clots of blood was in my stool as well as the toilet being filled/splattered with blood. She ignored the fact of all the blood and went to how my stool was ball size saying I was constipated and I should never come to the Er for constipation multiple times. I did not go to the Er for constipation. She explained to me like I was a child how stool should look and then went on to say the blood in my stool and pooping blood was very normal for my age, I would not die. I am a hair dresser as my nails are stained from hair color and she asked what was wrong with my nails and I told her I do hair for a living and they stayed stained. She told me people who were on drugs have the stained nails and I assured her I was not on drugs and she could test my pee and blood. She did a blood test for thyroid & diabetes. Coming in for my results the first thing she told me was "well I knew you wouldn't have diabetes because you are white and white people never have diabetes, y'all are so lucky" looking at my husband and I. Telling us how she was married to a white man and no one in his family has diabetes and they all eat lots of sugar and she doesn't understand how they don't have it, going on about how white people were so lucky and she doesn't know many with diabetes. After that she said blood was normal and the blood in my stool was normal. To then go on to explain to me how she thinks I have OCD because of my nails. I asked her is there nothing I can do for my bloody stool and to just ignore it? because I have had this issue for while now and it's getting worse. She told me yes there is nothing she could do about it that it could be many things and I will not die because I am young. Then telling me maybe for my headaches we could do a CT scan but she wouldn't recommend because of the radiation and that if we did do it and some crazy way she found a tumor in my brain there would be nothing we could do tonight for it so there is really no point and to go see my primary doctor. She explained in detail that finding a brain tumor doctors don't do brain surgery that night and she couldn't go in my brain and get it away tonight and it takes months or years before that. She never checked to see if I had a hemorrhoid, or tried finding any way to see if I have a fisher, no ct scan after telling her I wanted to do it she told us that the Er was extremely busy and it would take all night. We were the only people in the waiting room when we arrived. She did absolutely nothing. I will never take anyone to see Cartersville medical because of how degrading she made my husband & I feel for me going there for these issues. She was actually more interested in telling me about her Pyrenees/poodle mix dog she bought for $1, 200, her husband being white, and her son who is two years old than trying to figure out where all of my issues are coming from that are very hard to deal with not knowing how I can feel better. I was told by my doctor and a few nurse friends to report to Georgia medical board as she could be sending many dangerous cases to go home, I believe I am going to. This experience was very awful and I will never be back.

Desired outcome: Dispute bill with Patricia in ER. She was unprofessional and did not do anything to help.

I recently saw this PA at Wellstar Urgent Care. She is unprofessional and I wondered if she was actually even educated to be a PA.

I have never been treated so rudely by someone, much less someone in the medical field. I went to Wellstar Urgent Care in Marietta and waited 3.5 hours to be seen, then another 1.5 hours in a room. I finally went out of the room to ask for water and there was not a soul to be found. Finally, found a nurse who gave me a bottle of water. I was running fever, coughing, headache, body aches and after a series of tests this PA shows up talking quickly and loudly and asked what do I want from this visit? Do I want anti-biotics or steroids or both? Im not a medical professional. I rarely get sick. I told her that I just want to feel better. Unbelievable behavior.

Cartersville Medical Center Hypokalemia

I was put on Prednisone due to multiple bee stings. My potassium levels drop down to 2.0 and cause me to have to visit the emergency room. When I got to the hospital and was seen by doctor Katherine F Daniel's MD, she was very rude. She treated me like I shouldn't be there, that I was over reacting. Made me feel like a fool for coming to the hospital for treatment.

Cartersville Medical Center womack, matthew md poor care and attitude

My father-in-law has had open heart surgery for the replacement of his aortic valve in dec 2018. Since that surgery, he has had 6 strokes. Was told to immediately go to the er if any of warning signs were to come about. That the first 2 hrs are the most crustal but the first hr is the most critical. Matthew womack has now been the attending physician to my father-in-law twice. The first time he said he didn't believe that he was having a stroke. My father in law was later admitted due to neuro complications. After test were ran, he was found to of actually had a stroke at the time that dr. Womack was attending to him. Following the admittance to the hospital he then was later discharged to townsend park for rehabilitation. When seen by womack today, 04/11/19, the first comment made was that he had been there four times now and all the other times there was nothing wrong. Later sent him home without any discharge papers and told him it is all in his head!

Cartersville Medical Center employees

My husband was a patient this past week at this hospital the nurse staff on 2nd floor was the rudest most unhelpful people I've ever encountered in a hospital. We call today Friday 2-15-19 around 830am and speak to a Laura in administration and this lady is just as rude and unhelpful as the nurses there. This place has really gone down hill over the past few years with the staff they hire that is just so rude and hateful and acts as if they really don't want to be there and just hates their job so bad that they treat patients just any way they want to. Something really needs to be done with Mrs Laura and her smart mouth in administration and the nurses on 2nd floor. This past week HAVING to be a patient there was just awful. Thank you for your time Jamie Jolly [protected]

I was sent to the ER for treatment by my physician for complaints of chest discomfort, weakness and extreme sweats. In the 41 hours I was in the hospital I was NEVER seen by a physician, including my own from Harbin Medical Clinic in Cartersville. The hospital was dirty. The ER had dirty wash rags lying on the floor and caps off of medicine bottles and...

Cartersville Medical Center poor er care

my father was denied appropriate care and assessment on fri.july 3rd. he was complaining of blurred vision, elevated blood sugar (near 500), penial bleed and urinary retention. he had become agitated and combative due to elevated bs. they refuse to treat, , call police and one nurse said i have more patients to care for. i sought care from home health (whom helped) he went to a different er in rome georgia ( floyd medical er) whom sent him home via ambulance with no urinary output (my dad must wear an indwelling catheter for life) by time i got cmc er to except him(via ambulance) he was suffering with urinary sepsis. had he been treated earlier he might have not suffered so. i wish an investigation of the care giving be launched.maybe even class action. They also have reputation for releasing patients whom aren't ready for discharge.
