Monarch Dental - dental services - the form requires pre-authorization for unrelated services and is unfair

1. I went for my annual dental check up today (1/6/2016, 8:00 pm) and was asked to sign a standard form. This form asks us to agree to a clause which says asks us to authorize the dentist to perform procedures such as root canal if they felt it was necessary. If I was undergoing any surgery, I would absolutely see the need to authorize the dentist to change treatment plan mid course because I may not be in a position to give such authorization mid way through surgery. However, I see no reason to provide such authorization when I am simply trying to get my annual dentalk check up. The office person - Rochelle said that they would never actually do a root canal based on pre-authorization and wait until they had my express authorization. I then asked why she has a problem if I do not sign on a document which asks for pre-authorization. She said it was just a legal document, as though that is a reasonable explanation. It is precisely BECAUSE it is a legal document that I do not want to sign something I disagree with. She yelled at me and said I was just creating problems for her. Eventually I had to leave without receiving the service. The earlier time when I had simply gone for a dental cleaning, they wanted me to approve the fact that they might take Xrays. Since this was not even an exam by a dentist and simply a cleaning by a hygienist, I said that I did not see any need to authorize X-rays. Even that time she said that they did not plan to take any X-rays but they kust needed me to sign the form because it was the standard form. I refused to sign and after a lot of argument the office manager agreed to waive that clause. I understand the need for dentists to protect themselves and am happy to provide any reasonable authorization which matches with the service I seek. But to ask me to pre-authorize X-rays when I am just getting a cleaning by a hygienist or to pre-authorize ANY treatment such as root canal when I am simply wanting an annual check up is an unfair practice.
