Meta Bank/Bluebird review: Received debit card that was never requested

I randomly received a debit card in the mail today. Never applied for nor had I ever heard of either company. Want to make sure no one else gets a card in my name somehow. I want to know how they got my name and address and sent me a card without my permission. I am thinking it was opened in my name as an attempt at identity theft. I in no way ever have nor will I ever request an account through meta bank or bluebird.
