Iona Ambrose was taken to ER on October 8, 2022 because of confusion and high BP. Medical staff ran all the necessary procedures to find the cause. Everything looked fine. Discharged from hospital on October 10, 2022 back to Silver Creek Assisted Living. Arrived back there at approximately 2:00 pm. and then complained of chest pain and shortness of breath. They called 911 and Iona was taken back to ER. She had a heart attack. On Tuesday they suggested a Heart Cath to see where the blockage was. She had the heart Cath on October 12, 2022 and placed 2 stents in her artery. (Dr. Jones was physician) Dr. Jones did a great job of communicating with us.

Iona was doing well that evening, eating and talking and felt so much better. On October 13, 2022 in the morning i arrived to check on her and she was complaining about a pain in her left side. Then during the day she was placed in the chair in her room (2023) and apparently decided she wanted to get back in bed without notifying staff resulting in her falling out of the chair (which should have had an alarm same as her bed had) hitting her head (head was cut and bruised and a lump) Then it was downhill from there. On October 14 she was having difficulty swallowing, couldn't go to the bathroom resulting in her stomach becoming expanded and she was treated with lots of laxatives and to no avail (I know that in the the hospital doctor had mentioned bring in a GI but no one followed up. If they had my mother would not have suffered in pain due to not going to the bathroom. She also had issues with oxygen levels and was placed on oxygen. During this whole time she was never given physical therapy or getting her up and moving. Iona was in the bed for 2 weeks. (the amount of physical therapy was minimal and my brother was there to vouch)

On October 18, the social worker called and said Iona was being discharged back to Silver Creek Assisted Living. I was furious because she was in no shape to go back there. She had been bed ridden for 2 weeks and still had not had a bowl movement. I had to fight to get them to send her to rehab. Finally, my request was provided. Social Worker stated they called the rehab facilities that I suggested and she was finally going to Moultrie Creek as they had a bed available for her on Friday, October 21, 2022. However on this date my brother was told that she couldn't leave until she had a bowl movement. On this same day, I went up to the hospital and my brother and I told the day nurse that my brother was told she had to have bowl movement (Kimberly was the day nurse) she said that was not true that she had some movement one on Thursday (not significant) and they didn't have to go every day, but stated that Moultrie Creek didn't have a bed available (after we were told they did). So basically we received from the nursing staff that she couldn't go because of bowl movement and then we were told the rehab didn't have a bed.

October 22, 2022 I called Moultrie Creek and asked if they were going to have a room for Iona this weekend. The Director of Admissions Kami Geiger stated that they had a room for mom, and it was waiting on her arrival. The director also stated that when they know a patient is coming they hold the room. So, truthfulness on the hospital staff was not provided to myself and my brother. It was very disappointing. Iona had been in the hospital for 2 weeks and nothing was resolved and this should not have transpired. Our expectations of providing good care to every patient should be made. It shouldn't matter the age of person.

Most of my complaints are towards 2nd floor and 5th floor nursing. Iona was given the best care to the nursing staff on the 8th floor. I was thankful she was placed there.

I hope that this incident be taken seriously and investigated thoroughly so this horrible experience won't happen to another family. I appreciate you taking the time to read this complaint.

August 3, 2022 appx 12:10, after waiting almost an hour to see Shawn Wells for a 10 minute appointment, I was told, after signing a self pay paper of $92.00 (told when I made the appointment it would be $92.00) the staff decided it was now $153.00. They decided to make it a "well" appointment and charging accordingly. I refused to pay that amount and now I am writing this complaint. All I needed was medicine refills, which she didnt do.

Also I may add, that back in March of 2021, I was misdiagnosed at this location for a brain tumor.

My nmae is Craig Phillips, and after attending several visits to Dr. Northrop, a pediatrist in Palm Coast, he recommended bunion removal surgery, and suggested Flagler Hospital as the location for surgery.

I was told when paying $983 of copay at the door the morning of the surgery, that the copay would probably be 20% of the total bill. Since the surgery, Flagler Hospital has billed my insurance company $15,995 for the surgery, and I had to pay another $381 to the pediatrician. My insurance company paid all but $1,119.91, and the Hospital billed me for that amount. I tried and left messaged 3 different times for a hospital representative to call me, and never received a return call. Instead, the hopital just kept sending out the same bill, and now that bill has gone over to collections. AllI wanted to do is to see if someone at Flagler Hospital would think that paying over $17,000 for a bunion removal was very excessive. I feel as if I am another victim of this country's very poor healthcare system, and that I am over paying so that others can receive free treatments. My account number is [protected]. Friends took us to the ER because my husband needed an infusion for severe Sepsis. We were told very rudely that they would NOT treat him or admit him. No explanation given! I explained that he would die if he didn't get help ASAP because Sepsis is fatal and attacks your organs fast & you die. She again said no, and told us to get out and go home. I told her I'd go home and call 911…she said go ahead call 911 but we still will refuse him treatment. We were then escorted out. They are heartless and uncaring to treat a man who served his country for 9 yrs., they may have well spit on him, while they were at it. We have Medicare & federal insurance through Blue Cross, but they didn't even ask. They violated the EMTELA LAW, which is " you can't be refused treatment at an ER" even if you don't have health insurance! As for my complaint against them. It seems the CEO & others on the board don't care either. I'm told others recently died of Sepsis at this ER & hospital around the same time. 🤔 hmm. They violated the EMTALA LAW. Flagler Hospital in Palm Coast too has a very bad reputation. I was told to go to Baptist South Hospital where they treat you with respect & care.

I believe I was a victim of discrimination at Flagler Hospital ER on Monday, May 24th, 2021. I went in for help regarding my mental health issue of bi-polar. I needed help with my medication management because my regular psychiatrist was out of town until Wednesday. Starting with the lady that took me back to my room, the visit was a complete disaster. The room was dirty and not fit for a patient, the lady that took me to my room was rude and and couldn't care less about what I was there for. I had to explain my story at least 4 times. The doctor came in very quickly, did not itroduce himself at all, rolled his eyes when I told him what I was there for and said he would order labs and abrubtly left the room. The only one that seemed interested in my care, was the nurse Aaron. A crisis councelor came in and I told her my story and she worked on my case for a while. My labs came back saying my med levels were below level so I'm not sure what is going on. They did not help me any further, they gave me a 2 day prescription for Klonopin, and sent me on my way even though I expressed that I was probably crashing. The doctor never even came back in to talk to me. It was an awful experience from start to finish and I did not get the help I needed at all. I will never go back to Flagler hospital again. I was discriminated against for my mental illness. This should be stopped immediately and doctors trained on how to treat people like human beings. I don't plan on paying for this visit. Absolutely nothing was done to h elp me what so ever.

My mother has mystenia gravis, and a 9 year old pace maker, has never had to call 911 for an emergency. But while on Vacation she almost went into mystenia crisis. Immediately when the ambulance arrived the comment from one of the attendes was " im sure thats not the case" referri g to mystenia crisis. She was having trouble breathing her heart rate was going up to 120, and this woke her up at 2:00am... So if she has anxiety..it would understandably be due to the fact she knew something was terribly wrong. She had been told my her specialist to tell immediately if this ever happened about her conditions..that were brushed off, and they were just sure ..was not the case..
She was made to walk to the ambulance, as she barely could, and was extra stressed that she was not being taken seriously. She knowes That death is an outcome to mystenia crisis when your attendees are not familiar with your condition. The uncaring, passive, and sarcastic maker of all we came in contact with was horrifying. We left, I helped her out of the hospital, grabbed a wheel chair myself, and we Traveled back to our local hospital where she was taken care of. It was seriously a joke, she only had her bp checked, and a lot of judge mental uncaring looks. Avoid at all costs from our experience

My girlfriend and I came into Flagler hospital tonight to get checked out as I have been sick for over a month. We got in quick and we're ready to see what the issue was, hoping that some tests could be ran. Dr. Ward comes in the room and starts to asks a series of questions. Starting off by asking general symptoms and then he leads on to ask personal questions such as.. asking in depth why I don't have a job.. I go on to tell him I come from money and don't really need a job. He gets kind of rude and says he wishes he could not work. He asks me if I'm in school and I tell him no and explain further in about how I owe some money. He then asks me how I have money to not work but not pay for school.. which made me feel incredibly uncomfortable.. I shouldn't have to explain my personal issues at the hospital.. I'm here to be comforted at the very least! He then asks if I'm pregnant. I go on to tell him how my girlfriend and I have(accompanying me) been together over 2 years and it is not possible and how I do not want a pregnancy test because it is absolutely not possible. He goes on to tell me how "you never know" and how "you would be surprised"... and "you never know what could happen", this made me extremely uneasy and on edge considering the amazing and faithful relationship that I dedicate to my girlfriend. He then starts to head out of the room and asks me what I would like to do and then tells me that he will run a few blood tests but he doesn't "think" that there isn't anything going on. All in all, very odd experience so I decided to travel a little further and go to a different hospital in hopes that I won't get interrogated and rather get the medical treatment that I need. He made me feel more like I was on trial versus getting medical treatment. It's scary to think that this/these doctor(s) are assuming medical conditions and not properly taking action to figure out what type of illness the patient may have. I have heard plenty of complaints about this hospital but this is the first that I have ever felt extremely unimportant and mistreated.

Emergency room ct scan diagnosed as intestinal ischemia. Checked in about 10:00 am
So I'm in observation, can't get a room upstairs, because they are short on help, was told yesterday the 23 hour observation time was up so had to go to a regular room, that was about 2:00 11/2/2019

7:15 Nurse drew blood 7:30 Went to the bathroom, still blood from bowel, showed Japanese, or Chinese, nurse, she just ok. Went back to bed still not connected to Iv. Ice in water pitcher is water, told iv nurse needed ice, only allowed ice...It is now 9 am

The care received from the staff and doctors was excellent. But the er was filthy and is obviously not being cleaned. I voiced my concern to the doctor and nurse who said that this is a frequent complaint. I was told that the cleaning company had just been changed due to the poor performance and this new company is obviously not doing a good job. I am a former er nurse and a cancer patient. I have pictures that I am attaching. This has to change. It is appalling. Hospitals should be clean.

My partner checked into the emergency room with severe pain in his lower right side. He had a cat scan and was told he needed an immediate colonoscopy. He was told he had to drink two small bottles plus one gallon of the prep liquid. After the procedure the next day he was returned to his room at 5 pm. He had had nothing to eat (except clear liquids) for more than 48 hours. He was not even offered a sip of water or anything to eat after the procedure. We were told he couldn't leave until he was discharged by the doctor on the floor, which didn't happen until 2 hours later. During this time the patient next to him received 2 dinners and Barry was given NOTHING. He had given his history of heart disease and especially arrhythmia, and was not told anything about how to restore his electrolytes and potassium levels. This led to the worst arrhythmia he has ever had. He passed out in the middle of that night and hit his head. After complaining to the hospital I was told they would use this as a learning experience for their staff. Beware of Flager Hospital!
