Elmhurst Hospital Hipaa violation

My name is ani minassian.

On april 3, 1982 my car and ny transit bus crashed on steinway st. Astoria ny, the ambulance took me to elmhurst city hospital, i had six broken bones in my back, two broken bones in my right ankle, broken nose and cuts on my chin and nose, two right ankle operations one year i was on crutches and until now i can not do standing jobs and until now elmhurst city hospital does not give me my one year of medical records to proof i was injured. Elmhurst Hospital Issues with staff not allowing me to schedule ultrasound axillary soft tissue pol lateral

My doctor put in an order in the system on 7/14 for a mam and an ultrasound soft tissue axilla pol lateral. I had requested this to be done on 7/27 both. When I showed for the mam the ultrasound had not been scheduled but I was told it could be done after the mam. It was not performed.

My doctor had to submit a new order yesterday and when I called today 9/27 I was put thru a drill for staff to follow procedure for a radiologist to read the report before deciding to honor the doctor's request. I had mentioned this to the doctor and asked if she still wanted it done based on the mam report and she said she wanted it done.

I am not on hold waiting for approval to schedule this ultrasound and have lost 2 months of time. I talked to Carolina yesterday and she told me to call [protected]. I did and had to wait on hold with radiology for permission to schedule still on hold.

Elmhurst Hospital Mental health

I came in for an anxiety attack because I felt like I could not breath after having an argument and also to make a report against the person who I had the argument with. I was lied to by the police and EMS responders, instead of being taken to a normal er and being helped I was soon to be living one of my worst nightmares. I asked for help since I felt dizzy and like I could not breath at the moment. I was instead taken to a mental health er against my own will. They stated that they told me a mental health er which they did not say even once, or I would have not gone willing since that was not my issues clearly. When they saw that I also suffered from bipolar disorder they asked me questions, and everything seemed just fine. I was told by the Ems responders that I was in fact having an anxiety attack and nothing to do with anything else they can give me medicine for my anxiety which I never even got another lie told by them. They took me against my will to a place that I call "the nightmare from hell er". This place was completely unsensitized, blood everywhere, the bathrooms out of control. They even had me sleeping on dirty surfers and I was dined food at first and I nerved got any pads which I had to use tissue. I fell completely violated. I was forced into the room and attacked by the hospital police, doctor, and other staff members, and it was all uncalled for. I did nothing but record everything until they attacked me. They tried to delete my evidence but failed horrible I laughed so hard at their faces, but I thank GOD that I was able to press the lock bottom before they took my phone out my hands. My religious belief was mocked at, Lucas stated do you still believe in GOD now and that was completely uncalled for> Yes, I will always believe in GOD, in Jesus' name I pray Amen. This people should protect us not attack us and treat us like wild savage animals. I did not deserve anything that they did to me that night. My civil rights were violated. I was discriminated, attacked, and my gender was insulted. They should have their license revoked and criminal actions should be taken against them.

Elmhurst Hospital Lack of pt care

I am the emergency contact for a current PT. She has texted me numerous times about spending hours in her own feces because they are not changing her after she voids herself (she has numerous broken bones and is bed-ridden)

A nurse told her she should going in a bedpan (there is not a bedpan within arms reach) and cleaning herself up afterwards. One of broken bones is her pelvis so movement is limited and twisting is anot possible. How does this nurse think she could accomplish this task? The nurse finally gave her a bedpan and it's wrong one for the PT's injuries. It's too high.

They moved the PT to several different rooms and the last time left her for a week with the heater turned off in her room. PT complained numerous time about being very cold. Three blankets and one extra sheet later - no one thought to check to see if the heater was on!

Her nurse call bell is also broken. How can leave a bedridden PT in a room for over a week with NO way of contacting a nurse in an emercency? the rememdy that came 7 days later? a "desk bell". what kind of place is this?

Desired outcome: transfer PT. I had asked the first week she was there and was told (by the hospital administrator on duty)that if the hospital is and can provide the medical care - she cannot be transferred.

Elmhurst Hospital Lab

I saw my Dr (155 Brush Hill Elmhurst) she gave me 2 orders to have have blood drawn, so I went to the 4th floor lab to have blood drawn. I just received a test result, it was not the orders provided by my Dr! This is the second time this happened to me! How could she have missed the orders! Now I have to go back! Where and why would she run blood not from the order! This is unacceptable!

Elmhurst Hospital Appointment time

Everytime we have appointment they always call us too late its always 1hour 40 minutes late. It really has been too much. Please do something about it. Either hire more employees or make them work faster. Before i loved going to the hospital to see the babys sonogram because of the delay on every appointment i hate going to the hospital. Elmhurst Hospital Cable television

While currently sitting in Labor/Delivery triage I am appalled to come accross 2 stations one called Triggertimetv which is full of gun sales commercials and just watched a segment on active shooter drills. Normalizing and encouraging the disgusting gun culture should not be supported by a place healing. The other station is FOX news. Full of lies and conspiracy entertainment. Normalizing misinformation should not be a part of a place encouraging mental fitness. Dismayed and beyond disappointed.

Desired outcome: Remove or block access within the hospital to TriggertimeTv and FOX news please.

Elmhurst Hospital Billing problem

On 03/22/2022 I did my annual screening mammogram which is covered by my insurance because it is considered as kind of preventive medicine. Although the provider and the facility are in my net work, the insurance has denied the claim, they said that the provider 's name is missing in the claim (billing error).

While the hospital said is every thing is correct on their side, you should call your insurance. I received a bill from the hospital with the amount $ 1232.00.

Today, I have been told that I will be sent to the collection agency. Many calls to the hospital and the insurance but nothing was resolved.

Desired outcome: I would the the billing department to put my account on hold ,and then they should contact the insurance and find out what is missing in the claim.

Elmhurst Hospital Front desk employee

Today 03/29/2022 at 7:10 am there was only one front desk employee (man) was there. We were waiting to be called and he said something ‘where you guys went?’ After hearing that I had no answer and got confused. I paused for a minute because we just came from home for my mom’s culture at infectious disease center. Then the employee said ‘if you want to stay there then wait there’, He sounded very rude. I felt being neglected and felt racial discrimination. I still waited and After some time, I went there and asked nicely that we are here for my mom’s culture. The guys got furious and started saying why you guys are standing in the line and did not come to me earlier. I said when we came last time we waited to be called. And I did not know if you are ready to start. So I was being nice and waited there in the line. He was angry because we did not come to him right away. This guy is not a good person and not suitable for front desk operation. He is employed to help and guide visitor/patient/customer instead he raised his voice and was furious for no reason. May be something happened with him or his family but this is not the place to bring it.

if you know this guys or happen to have any issue file it so nobody get neglected or discriminated in the future.

Desired outcome: Please review our complain and rely our concern to him. The employee should know his duty and do his part. He needs to learn a good lesson.

Elmhurst Hospital Appointment

For the last 3 days I have been trying to reschedule an appointment. Every time they transferred me from one line to another line. Last time, they gave me a different number to call, phone #[protected] (because my referral is for a doctor outside of the h+h hospitals) but no one answer. I have been waiting for more than an hour; I tried to call a different hour always the same issue.

Elmhurst Hospital I have a complaint on the blood work department on the the 1st floor

The lady in the front desk very unprofessional not only was she not wearing a mask shes also eating snacks while talking to me or anyone that that comes in. She had me waiting to been seen and when I asked she told me I be seen when ever she feels like it very rude and the other guy in the front desk threatened on getting me kicked out the building for me asking questions explaining he kind of seem like a bully.

Elmhurst Hospital A nurse name Edna

Dr. Samuel Yunez has an nurse that works in his office by the name of Edna this isn't my first in counter with her I let Dr. Yunez know she was rude before he said he would talk to her but today another incident accrued where she was very rude again. My 1 year old son been sick and I've been trying to get him an follow up appointment like the hospital told me and this nurse told me doesn't need an follow up appointment and was very rude and refused to make him an appointment

Elmhurst Hospital Nurse

I was in the ER as a patient. September 2nd 2021 evening arrived 4.23pm I was Very weak and I had pain in my chest and arm and neck pain. When I was brought to station B1 a nurse in purple uniform around 7pm/8pm with short black hair and glasses. I asked her to help me to get out of the bed to go to the restroom. Her response was that I should do it by myself. And then she walked away. Several ppl during the 8 hours I was there have asked her for help and every single patient she turned down. Not to mention that she forget to put my name on my bed so the doctor couldn't find me and I was waiting there for 2-3 hours without anybody known I was there because of her mistake. Extremely incompetent

Elmhurst Hospital unethical behavior

My medical record # is: 5040435
My Dr. Nancy Tarlin ordered blood draw in April to be complete 8/9/2016.
I have a follow up appointment for results of said blood draw on 8/16/2019. I have been scheduled for 4 months.

This incident took place on 8/9/2019, at 10:07am in the Blood Draw office in the E wing. I had an appointment. It is absolutely disgusting the way you are treated in this room at Elmhurst Hospital. From the moment I walked into the Blood Draw room, I was talked to like I was nothing. I was first of all, yelled at for standing in the wrong place. The tone of this woman's voice was uncalled for. It's easy to just ask nicely for someone to move. Then, I was demanded to sit down many times, which I refused, only because there were many older folks and people who looked very ill that needed the seats. I had an appointment. Every woman in this office is nasty, so beyond anything I've ever experienced anywhere before. The worst attitudes. They are not professional in the least bit and actually speak poorly about you when you walk away. I heard this many times as people would check in. I waited for an hour, I didn't mind, I expected it. When it was finally my turn, a lady name Gwen took me back. She didn't have good bedside manner from the moment she demanded me to hang my things up and sit down. I'm not an anima and you can speak respectfully to someone. First of all, no one closes any of the sliding doors when they take you back. I watched at least 10 people have their blood drawn in front of me. Why are the HIPPA laws not enforced here? I had to stand up and close the door before Gwen started. I should never have had to do that. HIPPA laws are in place to protect me from incidents like these. My privacy. Gwen immediately stood still and sighed at me when I closed the door. What's the problem? I have every right to close this door and I certainly didn't inconvenience her in having to do so, I did it myself. Gwen was rough with tying the bandage around my arm. I understand you have other people to get to but being gentle goes a very long way. I have rights! I have the right to ask you if you're using the smallest needle - don't speak to me like I don't. I don't roll your eyes and don't call me baby... "Yes, baby." She replied. I also told her, please don't call me baby, which she didn't like. That's not my problem. She pushed and pulled that needle three times into my arm, twisting it. I had bruises all weekend long that made me look like a junkie. She is absolutely awful at your job. Just awful. Gwen, also shouldn't roll her eyes when I tell her I don't need her bandaid either. I don't have to take it. In all honesty, I just don't want to take one more second of being in her nasty presence. I wasn't rude to herm, I was direct. When I got up to leave, I told Gwen directly that she has no place working with people at a hospital. None. She at first asked me why, she told me, "I can't improve unless you tell me." So, I told her, nicely and directly... she is rough, she has no compassion and she is incredibly rude in her entire demeanor. I told her that she has no bedside manner. She then got an attitude with me, tried to intimidate me into walking out the door, like I'm some type of child. She's completely ignorant to HIPPA laws, it's very clear. Everyone in this office is. Gwen and her coworker have no business working in a hospital. None. And I bet my blood draw has disappeared, my doctor won't have it at my appointment on Friday because I bet she didn't turn it in, out of spite. It's wild how it hasn't been emailed to me yet, usually I get my results within two days. So, if for some reason they do not get to my doctor, I will come after this hospital. I will report you to the highest authority. Let it be known, let it be recorded that I will not pay for a repeat blood draw through this hospital if it doesn't reach my doctor. I will take this to news media if I have to. I know my rights, I'm not as ignorant as people like to speak to me like. Elmhurst hospital, you have a real problem with your employees like Gwen and all the women working at their desks in this room. They are rude, unprofessional and have no place working with sick and people who are probably dying. It's appalling. I recorded this entire encounter. You can hear how she speaks to me, you can hear how I directly spoke to her. I have every right to tell a hospital employee they are too rough and are rude. I have this recording saved. She absolutely did not care one bit about how she was rough with me or how she spoke to me. She condescendingly replied, "Oh, well excuse me! Excuse me." Really not compassionate at all and it just proved my point even further about her nasty attitude and why she does not belong in this field.

I expect my blood draw to reach my doctor. I expect to be reimbursed for this procedure. I expect to not be charged if Gwen did not submit my blood work and I have to repeat this process. I will not pay this hospital for the way I was treated at this appointment on 8/9. Someone needs to contact me directly to work out a reimbursement and to not pay for another blood draw.

8/9/2019 this incident occurred.

It was confirmed with Dr. Tarlin an hour ago, with her nurse that my blood work was NOT submitted. 4 months of waiting for nothing. Having to take time off of work for nothing. This woman needs to be held accountable for her actions.

Elmhurst Hospital nurse

July 30, 2019 i brought my mom to the emergency. 2 different nurse came to withdraw blood and failed to do so and just butchered my moms hand. One nurse was trying uncover the needle and poked her own hand. She then with an attitude left all the bloody gloves and needle on top of my mothers stomach and left her abandoned. My mom had a bile obstruction surgery recently and her stitches are still not healed on the stomach area. Putting all that bloody stuff on top of her stomach was the most irresponsible and unprofessional thing to do. The staff over here definitely needs proper behavioral and technical training.

Elmhurst Hospital emergency room

My father is diabetic and has kidney failure. He had a surgery on his ankle this past Friday. On Saturday he woke up deaf. He had only taken the medicine that gave him for pain and now he couldn't hear. So we called the number the provided on the discharge forms and they told us to bring him into the ER. We got there Saturday at noon. It is now Sunday 10:33pm and we're still in the ER. They doctors and nurses just keep saying that they're completely full and we waiting on a room to open up. He missed his dialysis session and just now took him to get some X-rays and catscans. My father is cold, tired, frustrated and in pain. Normally people are put on a bed and they have a bed#. Well, my dad is sharing that bed# with 2 other people and we're mixed with psych-patients. It's completely packed. No room whatsoever so why are they still admitting? I've been to other hospital, yes they're packed but they're more organized and don't give false wait-time. On top of that it rained yesterday and the emergency waiting area flooded. So where are family members suppose to sit/wait? I'm trying to keep my calm but they're too much.

Elmhurst Hospital nurse attention

My mom is a patient on the 6th floor, I asked the nurses to change her diaper because she had pooped, I waited 45 minutes and still she was not changed, Ms Hudson( nurse assigned to that room) stared that she had changed twice on that day already and that someone else should do it, nurses had been on the nurse station laughing and making jokes instead of taking care of patients, I understand change of shift at 7, but it is 8 o clock and they still on the nurse station without making any runs, hospital lacks attention towards patients, my mom does not walk or speaks, I called to complaints and I am still waiting for someone to call me!

Elmhurst Hospital financial counselor

I had some questions about the extra bill I received from the self pay package agreement I had and instead of been professional about it and explaining it to the best of her abilities or referring us to someone who can, the financial counselor- Alandra was rude, very unprofessional; even yelling carelessly saying "your self pay pkg was discounted already anyway." However, paying $4500 cash for a 2days stay at the hospital outside of my physician pay is not cheap! Nobody should be spoken to the way that she spoke to us even if the hospital stay had been free. Elmhurst Hospital is a good hospital and staff like "Alandra" should not be in a position to dent the image.

Elmhurst Hospital emergency room

I came to the emergency room feb 22, 2019 in pain from a fall and thought it was broken. I waited a long time in pain thinking I must find out. The doctor came found out what happen how I fell. He then said you must now walk to the other side of the hospital to get a xray and that was a long long wait. Aftter I was told walk back got lost but wait in a chair for him to see my xray. Finally they walk me to a bed to sit on and told me I sprain it that it will heal. I was still in so much pain the whole month of march thinking he told me it will heal but the pain was effecting me so much. March the whole month so much pain that I decided let me see a orthopedic doctor find out how long do it take a sprain to heal. He had me do a xray and said I dislocated it thats why the pain was like that. He even showed me the area on the xray I saw it. He started me on physical therapy and shots for the pain. I try to do it while at work but as of june 1, I am on leave of absence working on getting my arm back and range of motion back. I trusted this dr to give me the right treatment. I was just told its a sprain youre feel better soon nothing else. I will am so hurt, I lost all trust in this hospital never to go back. I am now out of work because of the treatment I got.
