Utstracom Um100c Wireless Modem / no mailing address provided for mail in rebate and no customer serivce

I bought a USB product at a cricket dealer store in Houston Texas. I was told that I had a mail-in rebate on the product that I purchased. Since this purchase on 2/19/08. I have not been able to mail in the rebate information because I can't seem to get the mailing address.
You www.cricket.com does not have a mailing address or direction on how to get the address. All the local stores in the Houston area that I have connected have not been able to give me the information, because I haven't be able to get through to anyone at the store, becuase the recorded phone program does not allow you to not only get the adddress. but fail to connect you to a customer serivce rep. Nor does anyone return your call.

All I want is the mailing address for mail-in-rebates. for the purchase of a utstarcom un100c wireless modem.
