I was referred for advanced degenerative disc disease L5S1 with bilateral lower extremities L5S1 pain. Chronic variable pain caused from the advanced DDD L5S1 documented early 2011 by specialist M.D. Referred for three steroid L5S1 injections at this pain management in Austin, Texas. I got gabapentin prescribed and Tens unit got. The gabapentin put me to sleep, and could not take during day. Used at night to sleep for marked c5c6 cervical spine DDD and marked c6c7DDD with spondylosis and disc bulge.This was diagnosed by xrays and MRI early 2012 by this pain place which I had appreciated. I was given two steroid injections in 2011 for heel pain, tissues type pain, both heels. The bilateral lower extremities radiculopathy that the pain management REdiagnosed me with, that heel pain not same type pain. I got those 2 steroid injections in heels first, then was going to get the L5S1DDD steroid injections.The heel steroid injections made pain alot worse when standing or walking, and lasted a long time. Never took any steroid injections after that. the gabapentin did nothing for my pain, except for sleeping at night. my back and legs, feet do not bother me at night in bed, pain will go away. i was sent for P.T. for feet pain with a plantar fascitis referral, which I was treated for and did not help. In January 2012 I had MRI done that showed osteoarthritis lumbar sacral, marked DDD L5S1, with discogenic endplates changes, indent thecal sac from L5S1 spondylosis, multi level lumbar DDD, multilevel lumbar degenerative facets, multi level central canal stenosis, and multilevel foraminal stenosis. Also the pain management diagnosed me with cervical radiculopathy. I got P.A. and medical assistant wanting to give me steroid injections on cervical spine and lumbar sacral spine even though documented bad side affect AND allergen from, and I made clear no more steroids. Got disgruntled these staff in reaction to my no more steroids injections. All said here is my only opinions. So next was P.T. for all the other conditions. I was sent with referral for arms legs, back and neck pain. Seemed odd no diagnoses, I asked if this was all I was needing to them and the P.T. I went to. They said info all in. Three days in the P.T. I left, as the ecercises significantly hurt my back and radiculopathy. I told P.A. at appointment I informed P.T. I had to leave and why, had severe feet spasms, back spasms and worse pain from and could not raise my toes up for five days after the last P.T. session. P.A. very sarcastically and tongue in cheek tells me to take more pills, including tizanidine that I had prescribed. OF COURSE you do not go back after worsened like that. AND I learned later that month my DIAGNOSES was NOT given to the P.T., as I saw paperwork from P.T. They only put fibromyalgia, that I also have but not what primarily sent for, and no wonder exercises made me worse. This was shady to say least. Doing treatments and diagnoses, and then throwing me under a bus to make out like nothing wrong, and to gym me up to be a manual laborer! I was sent to another P.T.at my request. This time I brought medical records. I got treated decent. However saw when I got P.T. records out that pain management put back pain in diagnoses1 no other info except, lumbar and cervical radiculopathies. So dumped me into a scam program again. The program had no bending or twisying or lifting or carrying. However apparently P.T.s I had were told to measure me first and last day bending, and if I bended a little more to have THAT equal, bending lifting carrying pain partially resolved ! Of course if you not bend lift or carry, and do low back exercises and activities not to aggravate symptoms, you will bend a little more from trunk or waist! IRRELEVANT! SCAM! Not p.Ts fault, the con at end.Also found out plantar fascitis was set up as well. The agenda was changed, and the P.T. put I did heel raises each foot on floor and each foot on steps at home and in sessions that I did not! Pain managed said I knwe and had all this and dumps me out like have nothing wrong?! I am certified disabled before started this pain management and they were hired gun type doctors and staff!. also they put significant pschyiatric disease and hx. in record, just cause I checked depression in initial paperwork. Do not have! AND have all my pain numbers in writing and was LOW to high number, depending on activities. Saw ten without meds and three with littered in documents! They had in writing pain levels and manipulated it! Shes crazy or cured mind games! I sent amendments to records and all ignored on all this! No longer there at this place!
I have on hand and stored my mris, cds of, and medical facts. Can also have mris redone but obviously cannot afford. The april, 25, 2012 mri showed uncovertebral joints disease with narrowed foramen and narrowed foramina at three levels all this, at c45, c56 and c67. I was not even told about these synovial joints. I was only told I had arthritis and ddd and disc bulge, and to come in and get steroid injections.This played pulled some strings and got this mri, only to do the steroids for the money. All said my own humble opinions.

I am author of above. I got records out right away from P.T. place and they obviously censored out original evaluation and recomendations, but it was IN records! Several pages.So I thought they had they did treat me for the DDD. But they used one page CHANGED initial evaluation, which made like they did not, even though they did. Exercises documented that I did were in there.

After I wrote this complaint I saw a page in treatment for l5s1ddd, cervical ddd, both with radicular bilaterally pain caused from discs, diagnosed in several med records and permanent. What i thought was just a regular comment page, from the third session, was actually a CHANGED entire evaluation and recomendation! It had no diagnosis about DDD at all, like I had no diagnosis in neck or low back. And the discs are cause of the radiculopathies. The doctor sent no other info in and so they not go by his records I BROUGHT in. They should have TOLD me. Instead they treated me for conditions and made out like they did not by passing last page out and third day evaluation to my clinics! Is not that what they call liable! In past tried to amend the records, had not seen switcheroo page three was. This place ignored and I have proof I sent. Anyway the records are worthless and incredibly unscrupulous!

The P.A. after I told her I had the severe feet spasms and unable to raise toes up and worsened low back pain and spasms with severe leg feet pain, all day these symptoms for five days after had gone to physical therapy, and had informed the physical therapy place I was not returning and why, THAT is when P.A.told me, Take more pills, and now you can go back to the physical therapy.She was extremely sarcastic and tounge in cheek. I told her I informed the Physical therapist, I was not going back and why, before she told me that. I found out a few weeks later that they had not had my diagnoses and unethical exercise program and expectations.The P.A. marked in my med record, She refused to go back to physical therapy.She later changed it to, bout of sciatica, as reason in record. After I asked for another referal as found out diagnoses not in. So record still had wrong info along with all the other con artist stuff they put in. She knew exactly what she was doing, trying to discredit me, hired gun type providers.Had insurance place I had in past their cronies. A real piece of work. Gee whiz, all said my ONLY opinions. Are facts!
