    Although we did authorize it, their was a trial period and it was from US bank and we were supposed to cancel it if we did not want it. What I am upset about is that I had to call to get the policy and they never sent a claim form and I have 3 appointments that should be covered and 2 more next week and on the following week, but we shall see.
