Johnny's Drive Up review: FDMS OKB fees 1

We closed our business in June. I cancelled the credit card and thought that I had done everything that I needed. Apparently I didn't. We were auto billed $21.90 on 8/04/15. We were billed in July also, $17.30 was taken out of our account on 7/06/15.

What do I need to do to cancel this account?
Our business was in Spanish Fork, UT. If you could reply I would really appreciate it.
