Checkers & Rally's - survey on the back of the receipt

My wife and I both purchased separate orders at Rally's on South Madison Muncie, in today. There is not a 4-digit store number on either one of our recipes. How are we supposed to fill out a survey if the receipt doesn't have the information that you need.? I would like it resolved or I'll just start going back to McDonald's again and not doing rallys ever.

Maaaaan! alright, the fries here are greeat! ...And the fry lovers double! even better! they should put the ala carte fries with the fry lovers double meal options, you got the chilly cheese fries, and the gourmet parmesan fries. Greeeeeat restaurant

I made it online complaint on August 29th 2019 I got no reply now I'm here on the 5th of September to have my burgers replaced and I'm not getting them replaced so I'm finding another complaint
