American Medical Association [AMA] Crozier/keystone center for occupational health, Susan Dormstein D. O

Hipaa and ethics complaint.
On 8/262020 our company arranged Covid test for all employees through this company and this doctor who signed the scrip.
A waiver was signed to allow the testing facility to share the results with the company. Share!
To this date 9/17/2020I as the patient Has yet to received the results. But we've learned that the company receive the results of our test on September 1.
the priority and loyalty of the doctor and medical staff at this facility is to the patient.
I have received nothing! I only agreed to share the results with the company but in order to share I must first possess the results!
This if anything is highly unethical!
If not in violation of HIPAA laws.

American Medical Association [AMA] Sunnyvale texas baylor scott and white hospital

My 77yr. old husband went by ambulance Monday morning the 24th. of August 2020 for severe dehydration. After 4 hrs. of testing transferred to a room. I found out by accident after over 30 hrs. there, he was in the Covid ward. My husband is very ill with COPD has been house bound all year, we have nurses come in. Not only did they not tell me he was in the Covid ward they were letting me spend all day Monday and Tuesday with him. When I told his respiratory therapist I was told to remove him at once. So at 10:30 Tuesday night I had to drive 80 miles (round trip) in the pouring rain to get him. No one would help me because they were mad about me taking him out. The Dr. told me that everyone that came in was put in the Covid ward until they tested negative. To me that is saying you don't have it lets make sure you get it! My children are convinced this was intentional because of the extra money they get for Covid patients. Now my husband and I both are quarantined for 14 days.

American Medical Association [AMA] U.S. Naval medical center portsmouth

Joining the USN in 1987, I became an EEG Technician, stationed at Portsmouth Naval Medical Center (PNMC). Upon reclaiming an Afrocentric name (1992) — I had a "perfect" corpsman record, and reenlisted — I was targeted for a "kangaroo" AdminSepBoard after I defeated them at the UCMJ. I'm still seeking reconciliation. Norfolk Naval Base JAG officer H. Hanna informed me they regularly were contacted by PNMC to "do their dirty work." The navy violated "double-jeopardy" laws. I have mounds of Official Docs proving my claims. I've currently gone into restitution overload mode by contacting multiple government agencies, my Congressional Representative (Anthony G. Brown, unfortunately for me, he's ex-navy), and I've been exposing the terrorism and travesty I've experienced to press agencies, "Invisible Wars," and "Protect Our Defenders." While in, I had numerous fellow members of Army also, telling me how the incident I experienced was normal. Race riots on ships was also divulged to me. My Afrocentric name even led a Blackfemale, legal representative tell me during the changing process-which I underestimated the gravity — that "this is going to blow them away." I had no idea what she meant. I even had two Blackfemales who were forced to convict me irregardless of how they felt, at the AdminSepBoard, voluntarily come and visit me at my clinic and tell me to try and talk the Base Commander. Reluctantly I did. The BC said to me "bull*hit" when I told him the base had a bad racial (antiKEMETism) problem. At my [personally requested] UCMJ, two base officers testified on my behalf, a Capt. Johnson and LCDR Sayers of the Rheumatoid Clinic. My last Evaluation from the base they produced, along with a civilian nurse (Brenda Murdock, who said the navy's problem with me was me being too intelligent for them) had me at "4.0" for REENLISTMENT, as usual. And most importantly, This is a pattern the navy engages in as expressed to me by a civilian female of then Rep. Albert R. Wynn's office. I was lucky I mad it out alive, as exhibited by the military overseeing organizations of: "Protect Our Defenders" & "Invisible Wars."

American Medical Association [AMA] Corn removed part way.

On July 23 I went to Dr. Paul Clayton's office at McKay Dee Hospital in Ogden Utah to get two corns revomed. They got one off good, but on the second one still hurts. It was his assistant who used a special cutting tool. With the money they get it should be done right. Dr. Clayton did look over this this work. Also, I would like the hospital to pay the bill.

This is my neurologists office. The receptionists continue to call my mother about my upcoming appointments. I'm 34 years old. I've been a patient for years. I don't know why they all of a sudden started doing so a couple of years ago, but I find it rather degrading. It seems to be an obvious violation of HIPPA policies which seems to be an issue running rampant in Bakersfield these days.

American Medical Association [AMA] Bunion removal

Dr. Laura Kishmin in 2016 performed a bunion surgery on my left foot.
I wore a boot for 6 months, walking became a problem when the boot came off. My balance was off my hips hurt and had to see a chiropractor for several months. My toes on the left have a space between the big toe, second and third, I had to get shoes one size larger. My balance is still off - walking takes an effort. In my opinion it was surgery gone bad. I attempted to have several
conversations on my bunion removal. Went for a second opinion Dr Aaron Chokan he looked at my foot and stated"how long has Dr Kishman been out of her residency" Dr. Chokan [protected]. My balance is still off and I can not grow a toe nail on big toe. Please, check her other surgeries-

American Medical Association [AMA] Dr Travis Maak University of Utah orthopedic center

Dr Travis Maak Performed a knee surgery Dec 17th 2015 to necessarily remove some screws and cartilage build up. During my checkup he informed me my Flu shots were out of date and he gave me one which was WITHOUT my consent. It is also conveniently not in my medical records which I have called his office numerous times with no return call about the situation. Also the state of Utah is auditing me for the medical tax credit I claimed for that year which I will be filing a complaint with insurance commission. Thank you for your time

American Medical Association [AMA] Complaint against a doctor

In February of this year, my husband went into the hospital for Day Surgery. He had a pain stimulator put in his back to control his pain, which was on Feb 19, 2020. the Doctor admitted him because my husband was in so much pain, that even a strong pain medication was not working. On Feb 20, 2020, in the morning he was paralyzed from then waist done and his upper body was very weak. The Doctor ordered a CT scan and they found a blood clot. Dr Kelly said that my husband was a great man I asked why and he said that my husband broke a blood vessel in his back and then he said that he should have put a drainage tube. That statement told me that it was him (Dr. Kelly) who cut the blood vessel. My husband was taken back into surgery on Feb. 20, 2020 to remove the blood clot, well my husband is still paralyzed. He has had rehab at Tirrs Memorial, and at Lafayette Physical
Rehab. So he has yet to come home. He will be home on May 8, 2020. He is still paralyzed. Dr. Kelly who is a Neurosurgeon in Lake Charles will not return my phone calls. I haven't talked to him since February. Now I have to have all kinds of people come into my home every day amid this COVID Virus because he still has to have therapy. Now I want Dr. Kelly to give me one million dollars because this is his fault and he knows it. But of course, he will deny everything. He will also say that this was the husband's fault. Well, he either answers to you or he will answer or he will answer to a court of law. If you want to call me my # is [protected].

I have been going to a dermatologist in Enfield CT. The practice is called Integrated Dermatology on Elm Street. At first I was seeing Lenore Crosson, an APRN for my symptoms of Psoriasis and Ecxema. We tried several things, but some of them were denied by my insurance company. We decided to try something else and she put it in to my pharmacy. It was denied again. Over 2 weeks ago, I went to the same practice, but saw another person, since Lenore Crosson was no longer there. I saw Kelly Mead, PA. She was not very friendly and told me that they would call me. She also told me not to make another appointment. I've been waiting for over 2 weeks and have not heard a thing! I still have the same problems and don't know what to do anymore! Can you help?

Is it just me, or why all all Medical areas sensitive to females staffed with females? While areas sensitive to males--such as scrotal/pubic area--are staffed with mostly females. I've recently had a Colonoscopy and their were three females in the room; all females. Hospitals should do a better job of having the names and photos of the directors plastered on their walls. When such hiring is one-sided as this, there needs to be accountability. Do men 100% staff any Examining Room when females get OB/GYN treatments? Complain and show "domestic violence" scars? Run and dominate women's prisons?

American Medical Association [AMA] Use of cpt code 99053

1/17/2020, ER visit for chest pain at 300am, Chest XRAY has billing for services 10pm to 6am from Texas Radiology Associates (TRA) services using cpt code 99053. TRA balanced billed for $215.00 in top of them being paid for radiology services. TRA is contracted to have radiology services 24 hours at Baylor McKinney. TRA is a in network provider and the 99053 CPT code is being used to bill for services that are excluded by insurance. Desired solution is to have TRA stop billing after hours services by having AMA to update CPT code 99053 to not be allowed to bill for after hours services when on-site 24 hours services are contracted for a 24 hour ER facility.

American Medical Association [AMA] Unethical behavior

Since 4/21/2020 I have been complaining to this doctor about some of the medication he prescribed. It was not helping my situation. When I made him aware of the issue he refused to change me back to what I was originally taking . He refused and asked me to wait. I told his receptionist that I was having a lot of discomfort. I made repeated calls to this doctor letting him know that the medicine was not working. I called one Friday frustrated and sick to hell upset and angry. I left a voicemail telling him I needed help please change me back, I was a wreck and I told him I did know what to do. I did state that I might go to the private hospital to get some treatment. A week later he drops me as a patient and refuses to give me the medication I need to continue life till I can find another doctor. all medications I take can not just be dropped and my Family Doctor will not prescribe them to me either they keep trying to send me to a behavioral hospital.

American Medical Association [AMA] A physician's unethical statement made to the public

On Wednesday, 15 April, Dr. Mehmet Oz appeared on Sean Hannity and made the statement regarding getting students back into school. "I tell you, schools are a very appetizing opportunity. I just saw a nice piece in The Lancet arguing the opening of schools may only cost us 2-3%, in terms of total mortality." This is OUTRAGEOUS, coming from a physician whose oath was to "do no harm." He should have his medical license revoked and NOW. Being on TV and promoting this kind of reprehensible nonsense makes him a danger to our society; I really don't care if he's a danger to himself. GET HIM OUT OF THE FIELD OF MEDICINE. My Mother was a physician of the highest integrity. and he is an insult to those in the medical profession who have spent their careers, saving the lives of their patients.

I have been a patient at LVPG for several years. They have been treating me for migraines. Despite taking my medication as prescribed by my DR & noted by my pharmacist, when I contacted them, I was rudely talked down to by Nurse Deidre A P, and refused to pass along the request. This was extremely unprofessional, and I feel, beyond her authority. This unprofessional behavior is unacceptable.

American Medical Association [AMA] dr. Craig Kuesel

Over the past several months I have been seeing a neurologist in Traverse City, Michigan by the name of Dr. Craig Keusel for chronic headaches the onset of which coincide with a head and neck injury sustained in January 2019. None of the meager treatment he has given has helped and in fact has made matters worse while he refuses to do anything but increase the dosage of medication that obviously does not work while also advising continued physical therapy which has made matters worse. He has also seen to it that I only have access to his medical assistant and will not answer any questions in person which makes it impossible to express my experiences firsthand. To the best of my knowledge he is still billing Medicaid for these non visits. It also appears his office is sharing my personal medical information with outside parties without my consent or authorization.

I was badly beaten almost three years ago and they wouldn't let me take a x Ray when I couldn't do the mri, and then called the police on me since they thought I was homeless and wouldn't let me read my rights. I've suffered because of this and the hoapital sraff were very rude and dus not go by any normal prodedures. I have problems with my eyebrows growing an dpossi me cerebral spinal fluid leak. Haven't been able to get any medical assistance from any.

I went to Jefferson general in port townsend. Again, the staff were very unprofessional except foe my we dr. He did say to have me do I x-ray. Though the nurse and 'team dr.' Who came in after i was wheeled back ro my room where they cut me off of the phone, and told me I wouldn't be seeing my Dr who had came in the first time - Who had me scared- were not any where near legit. And had me arrested for interfering with a medical facility.i then spent three days in jail, and again for no medical help. Until recently my side's of my face were bruised yellow and there was no way u couldn't look at me and see that there was some thing wrong. Even the local police stayed with me to get me signed in there and that was 45 minutes. No one was the but 2 other people.
I got more care and concern by the local police department than the hospitals.there is more to this and more unreal able behavior from people that we are supposed to trust. I just don't feel like do ing into it. I would like to find a lawyer for compensation, if any know of one, or one woyld like to take my cases. Something has to stop this out here. Im one of many with complaints of all three, jefferson 😣😙😚, mason and clallam county hospitals here in washington state. They are under educated and do not have up to date facility nor mannerisms and no one is overseeing them🤕, nor investigating them.
