Sedgwick Claims Management Services Short term disability

I filed for short term disability for Anemia and also, from injuries caused by a car accident 11/04/2021. The claim for Anemia was denied. It was due to the Nurse of my Hematologist not hearing me in regards to the severity of my fatigue April of 2022. I went on to continue the claim due to my injuries from the car accident. I had gotten MRIs of my neck, back, and right hip. I was referred to a Chiropractor, Physical Therapist, and Pain Management Dr. I have two herniated disc, and tear in a nerve of the ball joint of my right hip as a result of the accident. I suffered from chronic pain whether I sat or stood during the day. I constantly rotate using cold compresses and heating pads to control the pain as much as possible. I had a very hard time sleeping through the night because of the sharp pain from my lower back and hip. I had severe Bursitis and my butt muscles were extremely stiff because of pain. I could not walk at a normal pace. I didn't pass 7 of 10 exercises outlined in my Physical Therapy Plan. My job is a desk job. Although I was provided an ergonomics desk and chair, I still suffered a lot of pain from alternating with sitting and standing, because my back, neck, and hip were never at rest. I submitted short term leave. tI was denied. I appealed the claim, and provided what I could. The appeal was denied, based on solely on the notations from the pain management doctor.

He did not conduct a thorough examination and went against the clear findings of my MRI results. Sedgwick denied my claim siting it was based on their independent investigation. Reviewing my full medical records and the outcome of the denial, Sedgwick didn't not conduct a full investigation. The claim was denied based solely on the notes of the pain management doctor. Sedgwick acted Arbitrary and Capricious. The investigation was not just nor wasn't thorough. There are many missing parts in their investigation.

Desired outcome: I a thorough investigation done, which would include the report of my MRI findings and physical therapy notes. I want the appeal to be overturned, and paid out accordingly.

Sedgwick Claims Management Services Intentionally not paying a workman's comp claim

I suffered an injury by a patient in June of 2021. The staff felt it was necessary to send me to the hospital via ambulance. Banner sent all claims thru my private insurance, instead of thru workman's comp as they were advised. They took forever to pay the claims for the hospital. They still have not paid the ambulance bill and I am still receiving bills and late notices. The company I was injured with has contacted them 2 times and the excuse they were given was it "the bill was missed". The bill still is not paid.

Desired outcome: Just pay the ambulance bill like you were supposed to, that's all.

Sedgwick Claims Management Services My short term disability claims.

I filed a short term disability claim and Sedgwick Claims Management is handling my claim for my employer. I was denied! My first examiner gave me an extension on turning my paperwork in and it was turned in by the extension date. The examiner sent me appeal paperwork to submit and I followed her instructions. My doctor submitted my information on time and it's been 3 months. I have not received payment because they're still reviewing my claim. They keep extending my decision date.

I'm not released by my doctor but I had to go back to work on 9/9/2022 because I don't have any money coming into my home.

Desired outcome: My case approved and payment made so that I can pay my overdue bills.

I have lived at 558 S Roosevelt, Wichita, 67218 since May 1st, 2021.

I reported to the owner, Dr Jeff Hesse seeing cockroaches in the kitchen cabinets and on the counters, stove, etc from the first week I moved in.

Dr Hesse did not respond immediately, then dropped off some roach sticky pads and "roach motels", which filled within a few days. I asked him to get the place fumigated, and he did not do so.

It is now over a year and a half later, and the problem still exists, the roaches have multiplied & have now found their way into the bathroom and living room.

I keep a very clean house and constantly wipe down all surfaces, and still have to kill 10-12 roaches by hand with paper towels every time I come into the kitchen to prepare a meal.

Please, can you plan an inspection and cite Dr Hesse? This is inexcusable especially for a doctor. the roaches have infested this apartment long before I arrived, and he has done nothing.

Sedgwick Claims Management Services Jaqueline salas corazon's sedgwick representative I am teresita calasin sister of corazon cosipe

My sister Corazon Cosipe with claim #4A2207M6RVX-0001 who was injured on 7/18/22. Her Sedgwick representative is Jaqueline Salas. Jaqueline is so incompetent to do her job. My sister has not received any correspondence/s from Sedgwick to date. When I spoke to Jacqueline on 8/1/22, I asked her about Corazon's claim # because I told her that my sister has not gotten anything from Sedgwick. She claimed that she sent one at 906 Glenwood Ave. NE Apt. 2, Renton, WA 9 98056. I told Jaqueline that Corazon went to the old address, and there was no correspondence from Sedgwick according to the person in that address. According to Jaqueline the correspondence was sent back to Sedwick, and she promised me that she will resend it to 11460 SE 164th St., Renton, WA 98055. Corazon has not gotten a single correspondence from Sedgwick to date. I left voicemail and email requesting correspondence/s, but Corazon received nothing. I asked her through email what is Corazon's worker's comp rate, and never answer the questions. The documents I received from Worker's Comp Alaska was so confusing.

Desired outcome: Please investigate your employee Jaqueline Salas for incompetency. Corazon needs correspondence/s on 4A2207M6RVX-0001, her worker's comp rate.

STATEMENT OF DAMAGE CLAIM

AT&T contractors started digging up my backyard and my neighbor’s backyard (number 21410 Cimarron Pkwy) adjacent to mine, in early May. I am not sure of the exact date as it did not matter at that time.

There were two large ditches, one in my backyard and the other in my neighbor’s, right on the fence. I understand that two cable joints were being fixed.

On 6th June I switched the sprinkler system back on as it was switched off due to the work going on in the yard.

I discovered that there were damages at two places on the sprinkler pipes caused by the construction work by AT&T. I got one of the damaged sprinkler pipes repaired on 6/23/2022, as that was the earliest the repairman could come. The second damage needed exploratory work and the repair man said he would come later with a crew to look for it. Since then, I looked for it and found it. I am now waiting for the sprinkler tech to come and finish the repair.

I am attaching photos of the damaged pipes.

The cost of repair is as below, invoice copies are attached.

Brady Wilbanks, Claims Examiner, deferred this claim to Don Hatch Co, who in turn referred it back to AT&T, stating that it was NOT a Don Hatch contractor but an AT&T Contractor, by their letter dated July 27th, 2022.

I have not had any response to a reminder I sent to Brady Wilbanks on Aug 17th, 2022.

This business of "passing the buck" should stop, and my claim should be settled.

The telephone number for Brady Wilbanks is NEVER picked up and messages NEVER responded to.

This is the worst service ever.

Desired outcome: Immediate settlement of the claim

It's clear that Sedgwick's job is not to settle claims but to delay, extend and confuse claimants to the point they give up their claim. They don't do claim settlement, but rather claim deferral in order to save these large corporations from paying a cent.

On 6/27/22 the ATT employee was installing internet service into the house in which I was moving. He accessed the attic space via the opening in our Play Room which is a converted garage. Although he was being careful, his foot went thru the sheetrock of the ceiling and caused damage as you can see in the picture. I'm not sure but I think his name was Alex. I do not have an estimate on the repair of this damage. The handyman I use for these things is on vacation.

I have tried to get my account number, but my cell phone number is listed incorrectly on MY ATT.COM so I can't get into view my bill. I called customer service and after holding for 22 minutes, I was cut off. Perhaps you can find the account number!

I am sorry for the delay in responding to your letter, but I was combining 3 households into one location and have just had a moment to deal with this situation. I am renting the home with the damage so it definitely will need to be repaired.

Sedgwick Claims Management Services Cell phone insurance claim never paid

Had documentation going back and forth. Documents were requested that I provided, then never heard anything back. Tried several more attempts. Now the card has been changed to another card, I'm not sure the insurance company is in business any longer. I guess the goal is to drag it all out until company or benefits don't exist any longer.

Sedgwick Claims Management Services Unprofessional manager

I had a minor accident at a parking lot in June 2022. The problem is that the person who caused the accident was renting a car from Avis which handles claims through Sedgwick. At the parking lot the renter was responsible that they hit my door and gave me phone number to follow up. I filled a claim with Sedgwick and later I received an email that the renter stated that the accident was not completely his fault so they only gave me half the cost to repair my car. The person I talked to about this, named Mia Tilson, was super unprofessional, rude, always shouted at me while I was speaking politely. Then when I was giving her logic behind my statement she hung up the phone on me and did not call again. She also accused me of fraud and that the renter is telling the truth without reasoning. She even threatened me that if I don’t accept this half price payment they may completely deny my claim. I wanted to file a complaint against this person and this case that they basically did not process only because the renter’s statement was different. Sedgwick or any other company should not let their employees treat people like that just to save them money.

Desired outcome: Mia Tilson apologizing for her behavior with customers. Case should be processed rightfully.

Sedgwick Claims Management Services Rental car w budget

I rented a car on august 8th from Everett wa. I was given a totally different car than reserved. I was pointed to the back isle n told go get it. No one walked me thru it: I noticed about 7 hours later an old rusty dent ( I took a pic) then I returned it 2 days later and today received an email saying I wrecked it! U can tell by the very old rust that it’s very old! I have a pic: I want help ASAP resolving this! I did not do a single bit of damage to this car.

I have been injured now for 4 weeks. No word from Sedgwick concerning my pay. May have to call NH Dept of Labor Insurance Commissioner or get an attorney. I have a broken foot, and am out of work. Sedgwick Claims Management Services workmans comp claim

i have been trying to get a conformation for an MRI and physical therapy for 2 and a half weeks for an injury that happened on July 5th the person that was assigned to my claim has not returned any phone calls emails or text from me or the doctors that have tried to contact him. The MRI appointment i had has been canceled because they cannot get a conformation from my adjustor. meaning that i will have to wait another week or more to get an appointment to find out what is causing me constant pain with my elbow. i have been placed on restictive duty at work and had to take time off to take care of this problem.

Did not receive pay because Sedgwick said they didn’t receive a fax from my doctor. I tried to reach my examiner around 9:30 am. At 1:30 still never recieved any call back. So unprofessional! Everybody I talk to said they would try to get in touch with him but never happened. Not good business at all. After so many calls I was then told it may take 1-2 weeks. I’m out of work how is that suppose to work for me and my family? Something needs to be done. Not happy with this service at all. I’ve read reviews and so many other people are dealing with similar issues.

Sedgwick Claims Management Services Workers compensation claim

It's took a month and 2 weeks to have someone reach out to me mind you the only way they called was me telling them I as getting an attorney. Then when they call they asked every question I was already asked the day of I'm accident which I was told was for this specific reason. So it seems like nothing was even started on my claim. Then the lady tells me to call and email here by the following Monday to get an update on my approval status and when the check will be mailed. I've called my agent 4 times In the last two days going straight to voicemail saying she will be out of the office for a holiday weekend in 2021. Emailed here 3 times and haven't had any response. You can't talk to anyone else about your claim I was told so now I'm stuck here playing the run around game on my check. A joke of a company is what Sedgwick is.

Sedgwick Claims Management Services Out of Pocket expenses Case # [protected]-0001

Hello, Several weeks ago my case manager turned in my out of pocket expenses. For my disability claim [protected]-0001

My case since has been closed, I am still not working full timeI, able to work 2 days a week. I continue to have Breathing/Lung Issues. Latest Pulmonologist work restrictions attached.

My huge disapointment with Sedgwick is their communication. several times I tried to get ahold of my claims adjuster, no response from them. I've sent multiple emails and several phone messages. no response to either. I was wanting info on how to be reimbursed for out of pocket expenses.

Now I am locked out of my sedgwick account, there is no email address or way to contact them.

My case manager Rene was amazing all the way through my claim. we spoke of this issue several times during the case. Finally she suggested I send bills to her and she would forward them to my claims adjuster. which I did.

I have not heard back form Sedgwick.

Desired outcome: Communication::: I would like to be refunded my out of pocket expenses, Including a check which Sedgwic denied payment on to my bank. yes a returned check denying payment. Case # [protected]-0001

Claim # 4A22055CSNK I got my sister a storage in Dallas 404 Westmoreland just for her stuff to be damaged by rats that were big as cats it’s sad that she broke her neck to pay the storage ever month but they haven’t even attempted to pay her for her stuff, kids stuff just everything is ruined I think I will go ahead and call code compliance and have the place shut down I never seen anything like this. When you think your stuff is safe and your paying almost 300 a month just to start over PUBLIC STORAGE is nasty and Sedgwick is a cheap company that don’t do anything for anyone Then they only pay 250 for rats being in the storage please consider that rats carry diseases and she has small kids this is so hurtful to see her starting over

Had to pay all the upfront costs in getting my car fixed. Now, I've sent all the paperwork in with all my expenses and I'm being ghosted. Can't get an email response or a phone call returned. I'm out nearly $3,000 and no one at Sedgwick can be bothered with updating me on when I'll get the release letter or a check.

Sedgwick Claims Management Services Claim neglegence

Id like to keep this simple as can be

6/23 claim started with sedgwick with first examiner

6/24 new claim examiner assigned

6/28 work comp claim packed mailed by first examiner? (contained all the paperwork that had already been submitted

7/11 i reached out in the "communication center" section of my account

7/11 received a response saying no information evidence or paperwork had yet to be received

7/11 responded with the information that i had opposite of what the examiner claimed

7/11 received phone call from examiner stating again that there was no paperwork or information to which i again called the examiner out on false comments. The examiner changed her story around and said that the only document she had was a document claiming that i could return to work. Two weeks before this I contacted both of my physicians and their care teams requesting the "time off from work" documentation. I find it hard to believe that the examiner claims to have no info on my claim except some return to work document. I was hung up on. 30 mins later my claim was denied.

7/12 checked my Sedgwick account and my saved username and password had disappeared from my login page and wouldn't accept my login after manually entered. Good thing I took screen shots that depicted timeframes and conversations that also do not match up with the examiners story

7/14 Received the call from the nurse working with both physicians. This was in regards to the request i made for this "time off" document. How is it possible that the Sedgwick examiner had access to the answer i was looking for 3 days before this call with my nurse took place?

I've decided that I will no longer return to the company i worked for. This was a hazmat incident and my recent health conditions and the timeframe proves this without question. And i feel like i was completely ignored and the run around so that I would give up. I have no income for the past three weeks and i am being evicted from my apartment which has also been explained in detail to sedgwick as well as my employer. I am still facing medical appointments thru Aug 3rd to determine y diagnosis

Desired outcome: I would like to have three weeks of straight pay deposited directly into my account before 7/22/2022 so that i may stay in my apartment. then i will resign from the company
