From Feb o7- current, I have been on disability through my insurance company/ employer. With this doctor certifying me as being disabled. Information was sent April 16th to the insurance company for benefits to be paid up to that time. Since then I have seen doctor Rizvi on may 7th and May 21st. With another scheduled appointment for June 23rd. Due to the doctor being out of the office for the month. Since April 16th to this present time I have been in contact with the doctor, insurance company, and my employer. Information about my condition was sent to the insurance company to continue benefits after April 16th. Benefits were denied pending additional information that was requested from the doctor. The problem now lies with the office repeatedly saying that there is nothing that they can do regarding sending in this paper work and the items that are needed to process this claim with the insurance company. Olga has been informed of the severity of this issue that needs to be resolved. She has no sympathy of the issues now at hand. I was told that they had already faxed over the information to my short term disability. They told me that they have not received any of the necessary paper work for me to continue this disability. Due to the doctor not filling out the proper information that was requested prior to his leaving out of the country and by his staff not able to redirect this matter of importance to someone who could take care of this situation. I have been repeatedly been told that there is nothing that they can do, and that it is my problem. I have been begging them to have the doctor call me when he is available but I have not received any kind of response. Therefor causing the insurance company to not be able to process my claim which is still open and pending information. Who has informed my employer when asked that they see no reason to why I should not be able to return to work. Based on the information that has been provided and the lack of information that has not been received by the doctor. This in turn, caused problems with my employer. They are pressuring me to return to work, because of the lack of information that the doctor has not provided. At this time my job has been terminated, because of the lack of the information that has been provided from my doctor and his office. Therefore not allowing me to have insurance to cover further treatments, or to have benefits paid since April 16th.
