Taking money out of savings without me knowing overdrafted my account want refund

They signed me up without permission, over drafted my saving's account, want my refund, they tell you to go to www.cancelidtheft.com&fill refund form out, when you go there, there's no refund form to fillout.

I.D.Theft.com...same here.I needed a paydayloan to keep a check from going nsf.I did not get the loan and was debited 31.98 instead for theft protection that I distincly remember checking NO!needless to say both checks bounced, so I dont know what will happen from here.I called and cancelled I.D. Theft and I cant find the form.
