I had applied online for pan on 4th. Sept.2010(ACKNOWLEDGEMENT NUMBER - [protected]) but sorry to say that nothing tangible action has been done except withdrawal of rs.94.00 from my savings bank account no. - 365955 at ubi, salkia branch on 04/10/2010 for the purpose.
Since I have in acute need of pan card I would like to arrange an enquiry into the matter and do the needful.

Sir,
I have been allotted PAN No.ACGPD3230K without Photo In this regard I apllied for photo ID vide CSF No.[protected] dated 30-01-2010 (UTI, Guwahati, Assam) but the same was not issued due to discrepancy in date of birth.For meeting discrepancy I submitted a photocopy of Affidavit in support of my actual date oof birth i.e.12-05-1964 issued by the court of law and two copies of my passport size photograph (duly attested by a Gazetted Officer) and send the same to the following address on 25th September 2010 by Speed post:
UTI Technology Services Limited, (Government Service Department), Post Box No.20, Plot No.3, Sector-II, CBD-Belapur, Navi-Mumbai-400614.
In this connection severally communication was made over telephone No.[protected] (from Mobile No.[protected]) during January 2011 to July 2011, One gentle lady had attended the call and informed me that affidavit submitted by me is not acceptable (without stating reason though reason was asked by me), again, when call her over phone, I requested her to return my affidavit stating the reason for not accepting the same she advised me to send a letter stating that my photo ID may be issued on the basis of my earlier date of birth and e-mail the same for early issue of photo ID. Accordingly, I have submitted the same duly embosing my signature. Again I call her and asked about receipt of my e-mail, she had assured that she will arrange early to send the photo ID. But sorry to inform you that neither photo ID was issued to me nor any information/communication was received from their end (till today i.e. 01-05-2012) though more than 9 months have passed.
Please look into the matter and arrange issue of the same or suggest me the further course of action at my end in order to enable me to get my photo ID against my PAN No.ACGPD3230K.
