I was convinced by a telemarketer to sign up for a 'free' 45 day trial of Careington's Formulated Health Plan, which is not health insurance, but rather a discount program. For the next 2 months afterward, life was busy with a totalled vehicle, family illnesses, and the holidays. Frankly, I completely forgot about the trial.
Until I checked my past credit card statements, and realized that they had billed me the same day of the call, and I now had 3 charges on my card. I called their customer service--after 2 weeks of calling a couple times a day, finally someone answered, and told me that after the 45 day 'free trial' (IT WASN'T), I am locked into a 12-month contract. "Even if I never used the plan and never plan to?" "Yes, I'm sorry." Maybe the discounts are worth the monthly cost, maybe not. But if you do decide to give them a try, use a card you won't mind cancelling since they have the most awful cancellation policy I've ever heard of. Good way to get a customer to hate the company and give bad reviews wherever possible.

I am looking at Formulated Dental and the terms and conditions are clear. Its 12 months with autorenew. If yo cancell you have to pay the balance for the year, so you will need to send leter that you do not want to renew when that date comes.
They cahrge $50 enrolment fee 1 time and $14.58 per month, so that's about $175 per year plus the one time fee. If a family can not save more then taht during the course of check-ups etc athen it is not worth it, it they can then good deal in my opinion. I looked at BCBS dental and its $75 per month with cap on yearly benfits and $100-$300 deductable. Hope this helps anyone considering this...RS in GA
